2 Situatie Sinds de introductie van de multidetector computertomografie (MDCT) technologie, is het aantal patiënten die een CT krijgen met IV contrast geleidelijk verhoogd. De patiëntenpopulatie wordt steeds ouder, met meer comorbiditeit. De bijwerkingen van IV contrast blijven een bron van bezorgdheid (CIN).

3 1 Rationale Volume IV aangepaste contrast injecties voor specifieke CT-onderzoeken zou in het voordeel zijn van de patiënt. De vermindering van dit contrastvolume mag geen (of weinig) invloed hebben op de beeldkwaliteit.

4 Historisch gezien : CT met IV contrast vast volume voor volwassen patiënten. (100, 120, 150 ml) Een op gewicht gebaseerd schema voor het toedienen van IV contrast wordt minder gebruikt, maar wordt wel ondersteund in de literatuur.

5 DOEL Het volume IV contrast bij volwassen patiënten verminderen en de beeldkwaliteit behouden.

8 Material & Methods Nemoto Dual Shot Alpha CT contrast injector (Nemoto-Kyorindo, Tokyo, Japan). Software: Injectie Calculator (icalc) (Medicor, Rotselaar, Belgium) De berekening van het te injecteren volume is gebaseerd op : BSA, hartritme en de gebruikte jodium concentratie.

13 Material & Methods 100 patients had been scanned for a thorax-abdominal or abdominal CT with a fixed volume of 120 ml. The same patients have been scanned with a newly calculated volume on a Siemens Definition Flash scanner. The resulting CT images were scored by two experienced radiologists who performed an objective scoring ( HU densities ) and a subjective scoring ( visual anatomical observation ). Objective scoring (HU): descending aorta, portal vein, iliac artery, splenic and kidney parenchyma, inferior vena cava, femoral artery and in the air. Focus: venous abdominal phase

19 RESULTS Visual grading (subjective scoring) (1 0-1) - Good (1) 89 % - Acceptable (0) 7 % - Poor (-1) 4 %

20 RESULTS Image analysis of the patient-tailored CT examinations with an adapted contrast volume showed that a good or adequate image quality was retained. The majority of these patients had the benefit of a contrast volume reduction. A minority of the patients, obese with high heart rate, received a higher contrast volume compared with our standard fixed volume. how low can we go

21 CONCLUSION It is possible to lower the injected contrast volume for a number of CT examinations in the majority of adult patients. Timing injection speed. Cost-effective
