32 Blijft over inductie We identified 157 eligible RCTs (n = 31,085). Overall, the risk of cesarean delivery was 12% lower with labour induction than with expectant management (pooled relative risk [RR] 0.88, 95% confidence interval [CI] ; I(2) = 0%). The effect was significant in term and post-term gestations but not in preterm gestations. Meta-regression analysis showed that initial cervical score, indication for induction and method ofinduction did not alter the main result. There was a reduced risk of fetal death (RR 0.50, 95% CI ; I(2) = 0%) and admission to a neonatal intensive care unit (RR 0.86, 95% CI ), and no impact on maternal death (RR 1.00, 95% CI ; I(2) = 0%) with labour induction. 35 Consider the majority of our patients: low risk ( no diabetes, hypertension; normal fetal weight, intact membranes, BMI < 30kg/m²)? What maternal/fetal-neonatal outcome is relevant to compare term induction at 39 or 40 weeks and expectant management? Much has been written on caesarean section rate (CSR), less on other outcomes including stillbirth rate (SR), womens experience, sudden infant death syndrom and infant long term health (e.g. atopy and autism ).

36 Basically we face two scenarios. The first plays at the prenatal consultation offering induction or waiting. A randomized trial for planned induction versus non-intervention seems most fit to unravel the finale of this scenario. Irrealistic comparisons such as immediate induction versus waiting do not reflect clinical reality. The most recent meta-analysis included 5 original studies relevant for the week period (1).In one CSR is unknown, resulting in 1275 randomised patients. The most recent study dates from 2005, the rest from 1983 and before, none of the studies included prostaglandines ( only amniotomy and oxytocin). This makes these RCT s irrelevant to todays practice and underpowered : when a difference of 20 % in CSR is considered significant e.g from 20% to 18 % at least 6039 inclusions are needed ( power 0.8; α = 0.05)

37 What about stillbirth? In meta-analysis no stillbirths in the induction group were noted because groups were too small. A recent cohort study suggested lower SR for induction ( at 40 weeks 9.5/10000 versus 14.9/10000), in a prospective randomised trial ( power 0.8, α=0.05) inclusions are necessary (2). Numbers represent the minimum for intention-to treat analysis, about 1/3 of planned inductions enters spontaneous labour and vice versa. Results of such an RCT show the outcome of a policy of induction versus expectance, not actual induction versus non-induction. Cohort studies with comparators like spontaneous labour one week later are systematically skewed and non-representative for scenario 1. We do not know the finale of the first scenario and have no good arguments in favour nor against a policy of induction.

38 The second scenario is played in the labour ward when the woman presents as planned induction or spontaneously in labour. An RCT is not the answer ( randomisation not possible),better are cohort matched control studies, correcting for as many confounders as possible, with as comparator a patient on the same day, treated by the same team ( decisions concerning caesarean for non-progression and the interpretation of non-reassuring fetal condition are subjective and influenced by recent events).this compares actual induction versus spontaneous labor. CSR is higher for induction in studies from Europe ( 3,4), as opposed to American data ( 5), in all groups CSR is twice as high in the American studies. Womens expectations, empowerment by midwives, obstetrician s patience, medicolegal and financial factors make both sides of the Ocean incomparable. To my knowledge data on stillbirth and long term outcomes are unknown.

39 The lack of recent and relevant data makes that any decision can only be individual respecting the woman s values and expectations.

41 conclusion Induction is individual decision but not < 39 weeks Fetal monitoring: every 15 min for 2-3 min ( MAKE NOTE everey time: hour + frequency) = ctg, if feasable Oxytocin and amniotomy: more art than science, discuss with the woman Epidural: we are not suffering she is: respect this and inform Third stage oxytocin! PLEASE!!!
