3 Summary Acute appendicitis is the most common cause of acute right lower quadrant abdominal pain and for emergency abdominal surgery and the clinical diagnosis can be difficult. Traditionally, surgeons have accepted a high negative appendectomy rate in patients with suspected appendicitis, in order to avoid a high number of perforated appendicitis. Over the past two decades, transabdominal ultrasound (US) and abdominal computed tomography (CT) have been increasingly used in an effort to cut down both the negative appendectomy rate, and ill-advised delay, possibly leading to perforation and its sequelae. US examination with the option of additional CT significantly lowers the negative appendectomy rate as compared to the clinical acumen alone, without adverse effects on perforation rate or in-hospital delay. A major disadvantage of CT remains the considerable radiation dose which is especially important in children, young patients and pregnant patients. An alternative imaging technique to CT could be Magnetic Resonance Imaging (MRI), as this imaging modality does not use ionizing radiation. This thesis reports on the imaging findings of patients clinically suspected of appendicitis, with an emphasis on the value of magnetic resonance imaging in this group of patients. In chapter 2, we present the results of a nationwide survey; by means of a written survey the policies for the imaging management of patients clinically suspected of having appendicitis in the Netherlands were inventoried. A questionnaire was sent to both the departments of surgery and radiology of all 105 Dutch hospitals. Questionnaires were returned from 98 hospitals. It was found that in the work-up of patients suspected of appendicitis US or CT was performed in a minority of hospitals in 50% or more of these patients. In a large majority of hospitals it was carried out in less than 50% of these patients. There is a widespread variability in preoperative imaging regardless of hospital type. 140

4 Chapter 9 This survey shows that despite the ubiquitous presence of US and CT in Dutch hospitals, the preoperative imaging work-up in patients clinically suspected of having acute appendicitis does not reflect this, only being performed in a minority of patients suspected of acute appendicitis. The main reason not to perform imaging were lack of additional value over the clinical signs according to the surgeons and imaging used (US) was found to be too operator dependent. This study suggested that, when implementing a new technique in the diagnosis of appendicitis such as MRI, this technique should be accurate as well as reproducible and readily assessable. In chapter 3 the results of MRI in a prospective study in 138 patients with clinically suspected appendicitis is described. Sixty-two of the 138 patients had a histopathologically proven appendicitis. This study showed that MRI has a high sensitivity and specificity of 100% and 99%, respectively. MRI determined appendicitis in 63 patients, with one examination being false positive. Our data confirm the high (35%) negative appendectomy rate when an appendectomy is performed on clinical grounds alone. MRI avoided 22 negative appendectomies and 64 hospital days of observation among the 138 patients, resulting in an overall effect on the use of hospital resources of a net saving of 55,746. In chapter 4, the findings at MRI are reported in a prospective series of 36 patients with suspected acute appendicitis in whom the initial Ultrasound (US) findings were equivocal. MRI was positive for appendicitis in 8 patients. In the remaining 28 patients MRI showed a normal appendix in 22, an alternative diagnosis in 4. In 2 patients MRI did not show appendicitis, a normal appendix or an alternative diagnosis. In both patients, an appendectomy was performed on clinical grounds, and in both cases a normal appendix was removed. MRI had a sensitivity, specificity, and positive and negative predictive values of 100% in these 36 patients, in case the 2 equivocal MRI examinations were deemed true-negative. The negative appendectomy rate was 2/67 (3,3%) and the perforation rate was 8/65 (13%). This study shows that MRI is an excellent alternative for a CT scan as a complementary study when US is 141

5 equivocal in patients suspected of having appendicitis. In chapter 5, a pictorial essay, we describe the imaging findings of MRI in patients clinically suspected of appendicitis, providing an extensive differential diagnosis based on imaging findings and we describe possible pitfalls in the MRI diagnosis of appendicitis. The cornerstone in imaging of appendicitis is detecting an enlarged appendix with signs of periappendicitis. In chapter 6, we evaluated whether MRI can accurately diagnose or exclude appendicitis in pregnant patients with clinically suspected appendicitis. In a 3-year period, 12 consecutive pregnant patients, clinically suspected of having appendicitis, underwent an abdominal MRI study. In three patients MRI demonstrated acute appendicitis, which was confirmed at laparotomy and pathology. In 2 of these 3 patients US was equivocal. There were no complications and all patients delivered at term. Seven patients had a normal appendix on MRI, and in two the appendix could not be identified, nor could any secondary changes indicative of acute appendicitis be seen. All nine patients with negative findings for appendicitis had resolution of their symptoms and were regarded as not having appendicitis. In chapter 7 our study of the MRI findings of cecal diverticulits is described. Cecal diverticulitis is reported in 3% of patients clinically suspected of having appendicitis. Misdiagnosis may lead to unwarranted surgical procedures and hospitalization, because right-sided diverticulitis has a benign natural history and surgical intervention can be avoided in the vast majority of patients. This prospective study was based on five patients selected out of a group of 156 patients admitted to the radiology department for further evaluation. In all five patients right sided diverticulitis was seen as an outpouching of the right sided colon with associated circumferential wall thickening of the colon and surrounding inflammatory changes. An inflamed diverticulum with adjacent colonic wall thickening and surrounding inflamed fat are characteristic MR signs. In Chapter 8, we present the first report of the magnetic resonance imaging 142

6 Chapter 9 features of epiploic appendagitis, as it is a clinical mimicker of appendicitis, in two patients. MRI appears to be well capable of making the correct diagnosis of epiploic appendagitis, and it is important to differentiate it from appendicitis, because epiploic appendagitis is a benign self-limiting disease, that requires no further treatment. Conclusions 1. There seems to be an underuse of diagnostic imaging in the diagnostic work-up of patients clinically suspected of having appendicitis in Dutch hospitals. 2. Abdominal MRI is an accurate diagnostic test to detect or rule out appendicitis 3. Abdominal MRI is accurate in detecting an alternative diagnosis in patients clinically suspected of appendicitis 4. Diagnostic imaging in patients clinically suspected of having appendicitis has a beneficial effect on hospital resources 5. Abdominal MRI is accurate in detecting or excluding appendicits in pregnant patients. 6. Routine use of US, with the option of complementary MRI, performed in all patients clinically suspected of having appendicitis, significantly lowers the negative appendectomy rate without adverse effect on the perforation rate, in respect with the clinical diagnosis alone. Suggestions for further research The challenge in patients with clinically suspected appendicitis is to prevent unnecessary operations or diagnostic delay without adverse effects, such as perforation. To help clinicians in the decision making process, there are several imaging modalities available for this cause. Extensive research using different diagnostic imaging modalities has been performed in the evaluation of patients 143

7 with clinically suspected appendicitis, with most studies focused on US or CT[1-11]. MRI is hardly ever used, because it is considered as a time consuming and costful imaging technique and because of lack of imaging capacity. But improvements in MRI imaging software and hardware has made MRI more available for diagnostic imaging of moving organs, with ultrafast scanning times and as can be expected this is a continuous innovative process. Also, MRI has become more available and is nowadays more considered as a routine imaging modality that plays a role in more common diseases. Is there room for improvement for MR imaging in the acute abdomen? In this thesis a simple and fast imaging protocol was used in patients suspected of appendicitis, and improvements are possible. These improvements may concern different sequences, studying the value of contrast enhanced sequences or further shorten the roomtime. Further research in optimizing scan protocol is possible, such as studying diffusion weighted MRI (DWI). Multicenter trials are necessary to investigate the reproducabilty of our results. A considerable effort should be made to study the role of MRI in acute abdomen in general, as data are sparse, how does MRI perform in other acute abdominal entities, like intestinal ischemia. In the end, possibly three imaging techniques - US, CT and MRI - will be available and useful in imaging patients with acute abdomen. Further research can determine which techniques can be used and in what order, where, when, why and in whom. 144

9 Samenvatting Bij de beoordeling van een acute buik wordt in toenemende mate beeldvormend onderzoek verricht (echografie, computer tomografie (CT)) als hulpmiddel bij het stellen van de diagnose. Echografie en CT hebben bewezen dat zij bij patiënten met de klinische verdenking appendicitis betrouwbaar de diagnose kunnen stellen, uitsluiten of een andere verklaring voor de kliniek kunnen geven. Echografie is echter in een niet onaanzienlijk aantal patiënten niet goed te beoordelen en daarom niet afdoend, meestal door adipositas, door een retrocecale ligging van de appendix of een diepe ligging van de appendix in het bekken. CT heeft zeer goede sensitiviteit en specificiteit voor het aantonen of uitsluiten van appendicitis bij patiënten verdacht voor appendicitis. Het nadeel van CT is dat er gebruik wordt gemaakt van ioniserende straling met zijn beperkte, maar niet te verwaarlozen carcinogene risico s, zeker in het geval van zwangere vrouwen of jonge patiënten. Een andere vorm van beeldvorming zonder het gebruik van ioniserende straling is met behulp Magnetische Resonantie (MRI). Het doel van dit proefschrift was te onderzoeken of MRI een betrouwbaar, veilig en kosteneffectief middel kan zijn, als aanvullende diagnostiek bij patiënten met de klinische verdenking op appendicitis. In hoofdstuk 2 presenteren we de resultaten van een schriftelijke enquête gehouden in 105 Nederlandse ziekenhuizen op de radiologische en heelkundige afdelingen. Hierin werd het gebruik van aanvullende beeldvormende diagnostiek middels Echografie en CT onderzocht bij patiënten verdacht voor appendicitis. Uit 98 ziekenhuizen werden de enquêtes teruggestuurd. De enquête toonde aan dat in minder dan 50% van de responderende ziekenhuizen in 50% of meer van de patiënten aanvullende beeldvormende diagnostiek met echografie of CT wordt verricht. Het gebruik van aanvullende diagnostiek bleek onafhankelijk van het type ziekenhuis. Als voornaamste reden voor het niet verrichten van aanvullende beeldvormende diagnostiek werd vermeld, dat dit geen meerwaarde had ten opzichte van de klinische diagnose appendicitis en dat vooral de echografie te afhankelijk 146

10 Chapter 9 was van diegene die de echografie uitvoerde. Deze enquête toonde aan dat in een minderheid van de patiënten verdacht voor appendicitis aanvullende diagnostiek met CT of echografie wordt verricht. Hoofdstuk 3 beschrijft de resultaten van een prospectieve studie waarbij een MRI werd verricht bij 138 patiënten klinisch verdacht voor appendicitis. Bij 62 van de 138 patiënten was er daadwerkelijk sprake van een appendicitis. De studie toont dat MRI een sensitiviteit heeft van 100% en een specificiteit van 99% in deze groep patiënten voor het aantonen of uitsluiten van appendicitis. MRI toonde appendicitis aan bij 63 patiënten, met een fout-positief onderzoek en geen fout-negatieven. Deze studie toonde tevens de hoge negatieve appendectomie ratio aan wanneer alleen op klinische gronden zou zijn geopereerd, namelijk 35%. De MRI voorkwam 22 negatieve appendectomieen en 64 ziekenhuis-opname dagen ter observatie, hetgeen resulteerde in een kostenbesparing van 55,746. In hoofdstuk 4 worden de resultaten beschreven van een prospectieve studie van 36 patiënten uit een groep van 142 patiënten met klinische verdenking appendicitis. Deze patiënten hadden allen in eerste instantie een echografie gehad, echter bij 36 patiënten was deze echografie niet voldoende om appendicitis aan te tonen of uit te sluiten en werd vervolgens een MRI verricht. MRI toonde in deze groep alsnog appendicitis bij acht patiënten, een normale appendix bij 22 patiënten en een alternatieve diagnose bij vier patiënten. In twee patiënten werd ook bij MRI geen appendicitis, normale appendix of een alternatieve diagnose aangetoond en er werd bij deze twee patiënten een negatieve appendectomie verricht. Als deze twee MRI onderzoeken echter als negatief voor appendicitis zouden worden beschouwd, dan had MRI een sensitiviteit, specificiteit, positief en negatief voorspellende waarde van 100% als aanvulling op de echografie. De negatieve appendectomie ratio was 3,3% (2/67) en de perforatie ratio 13% (8/65). Deze studie toont aan dat MRI een uitstekend alternatief kan zijn voor CT als een extra aanvullend onderzoek, mocht echografie geen adequate diagnose geven bij patiënten verdacht voor appendicitis. 147

11 Hoofdstuk 5 is een pictorial essay. Hierbij beschrijven we de MRI kenmerken van appendicitis en geven we een uitgebreide radiologische differentiaal diagnose. Bij appendicitis is de belangrijkste bevinding het aantonen van een verdikte appendix met omgevende ontsteking in de mesoappendix. Ook de klinisch beelden lijkend op appendicitis, zoals omentum infarct en rechtszijdige diverticulitis, worden hierin beschreven. In hoofdstuk 6 evalueerden wij of MRI betrouwbaar is om appendicitis aan te tonen of uit te sluiten bij zwangere patiënten met de verdenking op appendicitis. Bij zwangere patiënten kan de klinische diagnose appendicitis extra moeilijk zijn. Dit komt door de veranderde positie van de appendix, toegenomen buikomvang, veranderde laboratoriumparameters en door andere aan de zwangerschap gerelateerde buikklachten. Tijdens een periode van drie jaar ondergingen 12 opeenvolgende zwangere patiënten met de klinische verdenking op appendicitis een MRI onderzoek. Bij 3 patiënten toonde MRI appendicitis aan wat bevestigd werd door operatie en histopathologisch onderzoek. In twee van deze drie patiënten was het echografisch onderzoek hieraan voorafgaand niet conclusief. Er traden geen complicaties op en alle patiënten bevielen a term van een gezonde baby. Bij zeven patiënten toonden de MRI een normale appendix en bij 2 patiënten kon ook met MRI de appendix niet worden aangetoond, maar waren er ook geen secundaire tekenen van appendicitis in de regio waar de appendix verwacht kon worden. Deze 9 patiënten werden niet geopereerd. Bij al deze negen patiënten waren de klachten van voorbijgaande aard, en werd dit beschouwd als dat deze patiënten geen appendicitis hadden of hadden doorgemaakt. In hoofdstuk 7 beschrijven we de MRI bevindingen van rechtszijdige colon diverticulitis. Rechtszijdige colon diverticulitis komt bij ongeveer 3% van de patiënten met de klinische verdenking appendicitis voor. Klinisch is het hier niet van te onderscheiden. Een foute diagnose kan leiden tot een onnodige operatie en ziekenhuisopnamen, omdat coecumdiverticulitis een aandoening is die spontaan 148

12 Chapter 9 herstelt, waarbij een operatie vrijwel nooit nodig is. Deze prospectieve studie is gebaseerd op vijf patiënten, geselecteerd uit een groep van 156 patiënten verdacht voor appendicitis, die waren verwezen naar de radiologie afdeling voor aanvullend beeldvormend onderzoek. In alle vijf patienten was rechtszijdige colon diverticultis op de MRI zichtbaar als een uitstulping uit het coecum, met lokaal een circumferentiele wandverdikking van het coecum en omgevende ontstekingsverschijnselen in het mesenteriale vet. Een ontstoken divertikel met geassocieerde colon wand verdikking en omgevend ontstoken mesenteriaal vet zijn typische verschijnselen bij MRI voor coecumdiverticulitis. In hoofdstuk 8 presenteren we het eerste artikel van de MRI bevindingen van appendagitis epiploica in twee patienten. Een appendix epiploica is een aan de serosale zijde van het colon vastzittend vetlichaampje. Deze vaak zware appendices hebben een fragiele vascularisatie. Door torsie, afknikken of rekken, kan er een veneuze thrombosering met daaropvolgende hemorrhagische infarcering plaats vinden. Dit geeft uiteindelijk aanleiding tot een lokale peritonitis. Indien deze gelokaliseerd is in de rechter onderbuik kan dit klinisch appendicitis nabootsen. MRI blijkt goed in staat de juiste diagnose te stellen. Dit is belangrijk omdat appendagitis epiploica een benigne spontaan genezende aandoening betreft, zonder noodzaak tot behandeling. 149

13 Conclusies 1. Er wordt relatief weinig gebruik gemaakt van aanvullend beeldvormend onderzoek bij patiënten verdacht voor appendicitis in Nederlandse ziekenhuizen. 2. Een MRI van de appendix-regio is een accurate diagnostiche test voor het aantonen of uitsluiten van appendicitis acuta. 3. Een MRI van de appendix-regio is een accurate diagnostische test voor het aantonen van een alternatieve diagnose bij patiënten met klinische verdenking appendicitis. 4. Aanvullend beeldvormend onderzoek in het diagnostiche traject, door echografie, CT of MRI is kosten effectief bij patiënten verdacht voor appendicitis 5. Een MRI van de appendix-regio is een accurate diagnostische test voor het aantonen of uitsluiten van appendicitis bij zwangere patiënten met klinische verdenking appendicitis 6. Het routinematig toepassen van echografie met de optie van een aanvullende MRI, verricht bij alle patiënten met de verdenking appendicitis, heeft een positief effect op de negatieve appendectomie en perforatie ratio ten opzichte van het klinische oordeel alleen. Aanbevelingen voor verder onderzoek Ook in de toekomst blijft het een uitdaging bij patienten met de klinische verdenking op appendicitis een onnodige operatie of ziekenhuisopname te voorkomen, zonder daarbij het aantal perforaties met alle daaruit voortvloeiende gevolgen te laten toenemen. Om de klinicus te helpen bij het diagnostische proces zijn meerdere beeldvormende technieken beschikbaar. Bij de evaluatie van patiënten verdacht voor appendicitis is al uitgebreid onderzoek naar het gebruik van echografie en CT verricht [1-11]. De waarde van MRI is maar weinig onderzocht onderzocht bij patienten met verdenking op acute buik aandoeningen. Dit komt omdat MRI een 150

14 Chapter 9 nieuwere techniek is, en omdat het wordt beschouwd als een tijdrovend en in de praktijk moeilijker te hanteren techniek. Ook bestaan er wachtlijsten voor MRI en is er in de dagelijkse praktijk minder ruimte en tijd om een spoed MRI te verrichten. Maar vernieuwingen in zowel de hardware als software van de MRI-apparaten hebben hierin veel verbeterd. Er bestaan nu ultrasnelle onderzoek sequenties en dit maakt het onderzoek meer geschikt voor bewegende organen en belangrijk, maakt het onderzoek minder tijdrovend. Hierdoor kan er veel makkelijker een spoed MRI worden verricht worden in het normale drukke MRI programma. Deze ontwikkeling gaat natuurlijk door en nieuwe sequenties, mogelijk nog betrouwbaarder en sneller zullen in de toekomst beschikbaar zijn en moeten natuurlijk ook op hun waarde getest en onderzocht worden. MRI is steeds meer beschikbaar en wordt meer als een routine onderzoek gebruikt ook bij buikaandoeningen. De vraag is of er ruimte is voor verbetering van MRI bij het gebruik in het geval van de acute buik. Dit proefschrift beperkte zich tot een patiëntengroep verdacht voor appendicitis. Alhoewel de resultaten zeer bemoedigend waren is het gebruik van MRI bij de patiënten verdacht voor appendicitis nog lang niet zo uitgebreid onderzocht als voor CT en echografie. Naast appendicitis, zijn er ook andere aandoeningen die een acute buik kunnen veroorzaken. Bij deze andere patiëntengroepen is MRI nog lang niet afdoende onderzocht. Uiteindelijk staan nu drie onderzoeksmodaliteiten tot de beschikking om patiënten met een acute buik te onderzoeken, echografie, CT en MRI. De vraag is welk onderzoeken bij welke patiënten het onderzoek van eerste keuze en eventueel in welke volgorde? 151
