2 VOORWOORD Dit is het 12 e Papendalsymposium dat zoals gebruikelijk in samenwerking met de 7 laboratoria uit Oost en mede onder auspiciën van Screeningsorganisatie Oost wordt georganiseerd. Het thema is nieuwe technieken die potentieel geïntroduceerd (zouden kunnen gaan) worden in het Bevolkingsonderzoek baarmoederhalskanker in Nederland. De twee gangbare systemen voor computer ondersteund screenen van dunne laag cytologie worden besproken door twee sprekers van de betreffende bedrijven. De resultaten van de MAVARIC-studie uit Engeland worden gepresenteerd. In deze studie zijn deze twee systemen naast elkaar onderzocht. Voorts zullen we een overzicht krijgen van de veranderingen in de nieuwe praktijkrichtlijn 3.0 met betrekking tot deze nieuwe technieken. Verder zullen we bijgepraat worden over de mogelijkheden van de gangbare HPV-testen. We hebben een keur aan sprekers uit binnen- en buitenland bereid gevonden om deze avond voor u te verzorgen. 5 Experience with the Hologic Imager Dr. A. Wilson Thinprep Imager The Scottish Cervical Screening Programme has funded a feasibility study into the use of the Thinprep Imager. The aim of the study is to assess the impact on service delivery of the Hologic Thinprep Imager (TPI) system within the Scottish Cervical Screening Programme. The study used a network model to assess both the use of TPI and the use of Multicyte the remote connectivity module which allows reporting of slides distant from the hub laboratory. The study splits samples into a manual arm and an Imager arm. The results of both arms will be compared. The Imager has been installed in six laboratories in Scotland grouped into two networks of three labs. One network is in a largely rural setting over a relatively large area and the second network is in a mainly urban setting around a large city centre. Over 60,000 samples have been processed using the Imager The study results will be presented as part of this presentation including a statistical analysis of the results and the logistics of using this new technology 5

6 Computer assisted screening using the BD FocalPoint GS System Dr. B. Tench Objectives: Guided screening of cervical cytology specimens is a significant advance over routine manual screening. In order to determine if use of the BD FocalPoint GS Imaging System (BD Diagnostics - TriPath) is more effective than manual screening, a large clinical trial was performed. Methods: A 2-armed, masked, adjudicated trial, using BD SurePath slides (BD Diagnostics - TriPath) was run at 4 clinical sites. The control arm (CA) consisted of routine manual screening and quality control (QC) rescreening. The experimental arm (EA) consisted of screening by the BD FocalPoint GS Imaging System, cytotechnologist review of up to 10 fields of view (FOV), with signout as negative or escalation to full manual screening, in addition to QC rescreening and pathologist review as appropriate. All positive, discordant, and a subsampling of negative slides were adjudicated to a reference diagnosis. The results obtained in the two arms were compared to the reference diagnoses and sensitivity, specificity, and negative predictive value (NPV) were calculated for ASC-US+, LSIL+ and HSIL+ groups. Results: 12,313 slides were evaluated. The detection sensitivities for HSIL+ were 85.3% (EA) and 65.7% (CA) (p<0.0001); and for LSIL+ were 86.1% (EA) and 76.4% (CA) (p<0.0001). For ASC-US+, the sensitivities were not statistically different between the study arms at 58.1% (EA) and 59.2% (CA). Specificities were slightly greater in the CA for HSIL+ (99.4% (CA), 98.7% (EA) (p<0.0001)) and for LSIL+ (98.0% (CA), 97.5% (EA) (p=0.037)), but slightly greater in the EA for ASC-US+ (96.7% (CA), 97.1 % (EA) (p=0.18)). NPV (CA/EA) for not-hsil+ were 99.4/99.7% and for not-lsil+ were 98.4/99.0%. Conclusion: Use of the BD FocalPoint GS Imaging System significantly improved the sensitivity for detection of the important categories of squamous intraepithelial lesion (SIL) and cancer with a much smaller decrement in specificity. Detection of ASC-US+ cases was statistically equivalent, and improved SIL+ NPVs show performance of the device improves accuracy for clinically important entities without increasing equivocal case detection. 7 Automation: Results of the Mavaric Trial Dr. M. Desai Background: Manual versus Automation Assisted Reading of Cervical cytology (MAVARIC) Trial is a Health Technology funded trial. It is a randomised controlled trial carried out in Manchester cytology laboratory in UK. It was designed to compare the manual screening and rapid rescreening of LBC Thinprep and Surepath slides with the automated screening technology available to screen these slides. Currently there are two commercially available automated screening systems which have been approved for primary cervical screening by the US Food and Drug Administration; the BD FocalPoint GS Imaging System (BD Diagnostics) and the ThinPrep Imaging System (Hologic). The FocalPoint system uses SurePath LBC slides and conventional smears whilst the Imager uses ThinPrep LBC slides. In addition, the BD FocalPoint SlideProfiler facility assigns slides by ranking them to quintiles and No Further Review (NFR). Quintile 1 contains the most abnormal cells and NFR is assigned to up to 25% of slides, meaning that they can be reported as negative without being seen by human eye. MAVARIC study was designed to compare the accuracy of these techniques in the detection of underlying disease. Methods: Cervical samples were obtained using Thinprep or SurePath LBC technology from women aged in Greater Manchester in primary care setting. Medical practices using different LBC systems were cluster randomised, with stratification by Townsend deprivation index. Slides received at the laboratory were randomly assigned (1:2) to either manual reading only or paired reading (automation assisted and manual reading). A final result was determined for each reading. Low grade abnormalities (ASCUS and HSIL) were triaged for colposcopy by HPV testing. The primary outcome was the sensitivity of automation assisted reading relative to manual reading in the detection of underlying CIN2+ or worse in the paired arm. The reliability of the No Further Review (NFR) facility of the FocalPoint system was also evaluated. Findings: The principal finding was that automation assisted reading with both technologies was less sensitive than manual reading in detecting CIN 2 or worse disease however, the results of NFR were promising. The result of NFR suggests that with the use of BD FocalPoint Slide Profiler, upto 20% of SurePath slides can be reported without being screened by cytotechnologists or pathologists. 9 . Nieuwe Cytologisch Peil: Richtlijn versie 3.0 Dr. F. van Kemenade Een richtlijn is nodig om de kwaliteit te borgen van het screenprogramma, via, conform de WHO richtlijnen, duidelijke afspraken. In Nederland komt daar nog een bijzonderheid bij, namelijk dat er relatief zoveel verschillende laboratoria screenen voor BVO baarmoederhalskanker. Uniformiteit is in het screenen essentieel. De richtlijn is monodisciplinair en gebaseerd op een mix van evidence based - en consensus based criteria. De richtlijn focust op BVO cervixcytologie, maar is ook van toepassing op indicatieve cytologie De nieuwe versie legt meer nadruk bij de test en minder op de organisatie in een laboratorium, omdat er steeds meer pathologielaboratoria een kwaliteitssysteem hebben. Het heeft een flexibelere format gekregen waarin zonodig per hoofdstuk gereviseerd kan worden (een losbladige of modulaire opzet). Dunnelaagcytologie en computer ondersteund screenen staan er expliciet in vermeld. De nieuwe versie omvat nadrukkelijker dan te voren het zogenaamde vervolgtraject, waar immers een kwart van de potentiële effectivieit van het BVO afhangt. Daarom is ook de aanpalende HPV triage testrichtlijn recent definitief vastgesteld. Omdat zoveel verschillende laboratoria deelnemen aan BVO inclusief vervolgtraject, dient de registratie van dit traject eenduidig te zijn. De voor dit traject relevante lichte afwijkingen zullen analisten meer zelf gaan afwerken. Dit heeft ook consequenties voor herscreenen en multiplescreenen. De terminologie moest worden aangescherpt omdat veel termen, zoals rescreenen en kwaliteitsborging, vaak verschillend werden opgevat en toegepast. Nieuw zijn verder de carcinoom audit en, tbv de reproduceerbaarheid van de diagnostiek, het bijhouden van zogenaamde discrepantieprofielen. Het is belangrijk te beseffen dat deze richtlijn geen wet is, zoals de CLIA dat in de VS bijvoorbeeld wel is. Maar onze betrokkenheid en professionaliteit mag er niet minder om zijn. 10 HPV de toekomst van het nieuwe BMHK: Facts or Fiction Dr. W. Melchers HPV veroorzaakt baarmoederhalskanker. Met andere woorden baarmoederhalskanker ontstaat niet zonder de aanwezigheid van HPV. De humane papillomavirussen vormen een heterogene groep van kleine DNA virussen. Op dit moment zijn er meer dan 140 verschillende typen HPV geïdentificeerd. Hieronder bevinden zich een 30-tal verschillende HPV genotypen die het genitale gebied kunnen infecteren. Een aantal HPV typen zoals HPV 16, 18 en in mindere mate HPV 31, 33 en 35 zijn geassocieerd met maligne aandoeningen van de baarmoederhals terwijl andere HPV typen (zoals HPV 6 en 11), vooral in benigne aandoening, zoals genitale wratten, worden gevonden. Vrouwen die geïnfecteerd zijn met een zogenaamd hoog risico HPV type hebben een grotere kans op de ontwikkeling van baarmoederhalskanker dan vrouwen die geïnfecteerd zijn met een laag risico HPV type, of die niet geïnfecteerd zijn met HPV. Behalve baarmoederhalskanker veroorzaken deze hrhpv typen ook afwijkingen of premaligne voorstadia van baarmoederhalskanker. Met het aantonen van de hrhpv typen in baarmoederhalsuitstrijkjes zouden dus vrouwen kunnen worden geïdentificeerd die ofwel afwijkingen aan de baarmoederhals hebben ofwel een hoger risico lopen op het verkrijgen van deze afwijkingen. Het bevolkingsonderzoek vindt momenteel plaats door een cytomorfologisch onderzoek van een baarmoederhalsuitstrijkje. Ondanks de sterke reductie van baarmoederhalskanker die heeft plaatsgevonden na de invoering van deze screening, is de sensitiviteit van de cytologische screening tot op de dag van vandaag een discussie item. De sensitiviteit van de screening zou wellicht verbeterd kunnen worden door hrhpv detectie te incorporeren in de screening. Een aantal grote studies hebben inderdaad aangetoond dat detectie van hrhpv gevoeliger is dan cytologische screening. Dit houdt in dat hrhpv wellicht als primaire screening binnen het bevolkingsonderzoek kan worden ingebracht. Echter, HPV infecties komen veelvuldig voor en zijn zelfs de meest voorkomende seksueel overdraagbare infecties. Het virus is voornamelijk seksueel overdraagbaar en 50% van de vrouwen wordt binnen twee jaar na hun eerste geslachtsgemeenschap geïnfecteerd met HPV. Gedurende haar leven is het risico voor een vrouw op een genitale HPV infectie 80%. Hoewel HPV een essentiële factor is voor de ontwikkeling van baarmoederhalskanker nogmaals, zonder HPV kan er geen baarmoederhalskanker ontstaan - is de kans op kanker zeer klein. De infectie is wijdverspreid en er is nauwelijks iets bekend over secundaire factoren geassocieerd met progressie van HPV-geïnduceerde afwijkingen naar invasief baarmoederhalscarcinoom. Door gebruik te maken van heel gevoelige hrhpv testen is het mogelijk op virusmolecuul niveau te kunnen detecteren, met andere woorden iedereen die met HPV is geïnfecteerd wordt geïdentificeerd. De grote vraag is, gezien ook de hoge prevalentie van het virus, of hiermee de doelstelling, namelijk identificatie van vrouwen met baarmoederhalsafwijkingen dan wel het risico daarop, wordt gehaald. In Nederland is, afhankelijk van de leeftijd, 10 tot 30% van de vrouwen geïnfecteerd met HPV. Het risico op afwijkingen wordt echter niet bepaald door alleen de aanwezigheid van HPV maar veel meer door de persistentie, het langdurig aanwezig zijn van de virusinfectie. Tachtig tot 90% van de vrouwen klaren de infectie binnen een jaar (transiente infecties) en slechts bij een klein deel van de geïnfecteerde vrouwen persisteert het virus. Er is dus een duidelijk verschil tussen de technische en de klinische gevoeligheid van een HPV test. Een test die dus met name infecties kan identificeren die klinisch relevant zijn, is dus bepalend voor het succes van hrhpv incorporatie binnen het bevolkingsonderzoek. Dit houdt feitelijk in dat er een delicate balans moet worden gezocht tussen de sensitiviteit van een test, het aantonen van afwijkingen aan de baarmoederhals, en de specificiteit, alleen die vrouwen identificeren met een klinisch relevante infectie. 11 onderscheidt je nou de feiten van de fictie en is er hiermee een toekomst voor HPV detectie binnen het bevolkingsonderzoek mogelijk? Dit complexe onderwerp zal tijdens dit seminar besproken worden. 11

12 Diagnostic Performance of p16/ki-67 Dual Stained Cytology for the detection of Cervical Disease: Results from the Palms Trail Dr. H. Ikenberg Background: The detection of p16 over-expression is an indicator of cell-cycle deregulation and thus for the presence of cervical intraepithelial neoplasia (CIN). Occasional expression of p16 in normal cells can compromise that approach. Additional staining for the Ki-67 proliferation marker within the same cell may enhance specificity of the test. Objectives: A prospective diagnostic study was conducted in five European countries to assess sensitivity and specificity of p16/ki-67 dual-stained cervical cytology for the detection of CIN2+ (cervical intraepithelial lesions grade 2 or higher) in primary screening and in ASC-US (atypical squamous cells of unknown significance) or LSIL (low grade squamous intraepithelial lesion) triage settings. Methods: women attending routine cervical cancer screening visits were enrolled. Pap cytology (partially conventional, partially liquid based), HPV, and dual-stained cytology testing were performed and all women with any positive test result (except for HPV positivity <30 years as the only positive test) were referred to colposcopy/biopsy followup. Pathologist majority consensus diagnoses on biopsies served as gold standard. The diagnostic performance of dual-stained cytology was evaluated and compared to Pap and HPV testing. Results: In screening, sensitivity of p16/ki-67 dual-stained cytology for CIN2+ was found significantly higher (90.1%) than that of Pap cytology (66.4%). Specificity was identical for both tests (95.3% vs. 95.4%). The sensitivity of HPV testing was 96.4%, but with a substantially lower specificity (90.2% over all ages) as compared to the cytology based tests. In women aged <30 years specificity of dual-stained cytology was 92.3% compared to 81.4% for HPV testing. In ASC-US and LSIL triage dual-stained cytology showed high sensitivity, while reducing the number of false-positive results by 43% vs.hpv testing. Conclusions: Dual-stained cytology testing was shown to combine both high sensitivity and high specificity for the detection of women with underlying CIN2+ in primary screening and for the triage of Pap cytology results categorized as ASC-US or LSIL.
