14 Betekent een ehealth IC nu dat alles direct zichtbaar is voor de huisarts of andere zorgverstrekkers? NEEN (enkel) deling:gebruik in kader van continuiteit van de zorg Nog niet elke bron deelt alles of kan alles reeds delen wat hiervoor nuttig is (bv pacs en ) Patiënt kan steeds aan een behandelende zorgverstrekker vragen om bepaalde contacten of resultaten te verbergen (in webportaal CoZo) Als je als patiënt een zorgverstrekker excludeerde op ehealth site, dan kan deze aan niets van deze patiënt, zelfs bij therapeutische relatie 14

15 Heb je dan als patient via ehealth een volledig beeld op je dossier zoals via wettelijk voorziene inzage? Neen Lang niet elke bron ondersteunt rol patient, ook interhub nog niet ok Elke bron kan op basis van rol filteren (bv rol patiënt in UZ = geen voorlopige brieven of sommige resultaten niet etc) ehealth restrictiever dan patiëntenrechten (bv. scope, volledigheid, directe stop na overlijden, ) Modaliteiten voor patiënt via CoZo : >> onbekend nog bij publiek & artsen. ± 500 resultaten /maand effectief door patiënt opgevraagd Rechten uit wet patientenrechten blijven uiteraard: los van ehealth bv via ombudsdienst ganse dossier opvraagbaar (papier/pdf) bij elke arts/instelling 15

22 Coding systems LOINC (one code, one concept) vs SNOMED (pre/post coord) The case for LOINC: Covers most lab concepts Most important axes covered (analyte, specimen, scale, kind of quantity (method)) Problems with LOINC: Too much (eg 68 times glucose, 734 starting with glucose, concepts) Too little sometimes (eg calculations, specialised hemato, some method specific things eg for creatinin) Code + specimen + unit (Kind of Quantity) detailed unit: UCUM vs SNOMED (relationd) 22

23 Logical Observation Identifiers Names and Codes (LOINC) A unique code (format: nnnnn-n) is assigned to each distinct concept 6 main fields for every code: Component- what is measured, evaluated, or observed (example: urea,...) Kind of property- characteristics of what is measured, such as length, mass, volume, time stamp and so on Time aspect- interval of time over which the observation or measurement was made System- context or specimen type within which the observation was made (example: blood, urine,...) Type of scale- the scale of measure. The scale may be quantitative, ordinal, nominal or narrative Type of method- procedure used to make the measurement or observation 23
