23 Casus 1 Man, 54 j Verloop: Opname op 25/03 owv harttransplantatie Aspergillus waarvoor op IZ gestart met cancidas 10/06: transfer van IZ naar hartbewaking 17/06: switch cancidas naar voriconazole (Vfend ) Dosis Voriconazole siroop: 2 x 400 mg /dag (opladen: 2 x 6mg/kg, daarna 2x4mg/kg) 21/06: spiegel voriconazole niet detecteerbaar 23 23

24 Huidige medicatie: zovirax, cellcept, prograft, durogesic, duovent, ezetrol, medrol,pravastatine, redomex, serlain, tradonal, zantac Interacties OK Sondevoeding? JA Wordt gestopt 2u voor en 1u na toediening OK Voorstel infectio: dosis ophogen naar 2 x 600 mg /dag 24 24

25 Advies apotheek: Op IZ kreeg patiënt rifampicine van 23/05 tot 10/06 Major interactie met voriconazole: Rifampicine = CYP3A4 inducer Voriconazole wordt gemetaboliseerd via CYP3A4 Voriconazole concentratie in bloed oiv rifampicine T1/2 van CYP-enzymen = 1 week Effect van inductie zelfs tot 14d na stop rifampicine mogelijk!! Voorstel: dosis voriconazole 2 x 500 mg /dag 25 25

26 Casus 2 Man, 69 jaar, opname radiotherapie 5/02/ /02: heelkunde voor tongca 10/02: tracheotomie; diagnose slokdarmca Naar IZ 18/02: PEG-sonde 20/02: spontane evacuatie etter langs tracheotomie; geen fistel; geen onderliggend abces; empirisch start piperacilline/tazobactam Naar verdiep 22/02: associatie vancomycine owv MRSA uit kweken 26 26

30 26/02 Evaluatie vancomycine therapieën voor MIT Dag 4 en nog geen spiegels Schema 4 x 500 MG/1uur Downgrading naar cotrimoxazol aangezien MRSA en Enterobacter cloacae gevoelig? Op 27/02 gaat ASO langs op afdeling Klinisch Patiënt kan geen medicatie PO nemen. Parameters goed; geen koorts meer. Rondom tracheotomie nog wat inflammatie, maar wel beter. PEG-sonde. ADVIES Stop piperacilline/tazobactam (4 x 4g) (44,64 euro) + Vancomycine (2g) (33,24 euro) Start cotrimoxazol 2x2 ampulles/dag IV (3,94 euro) gezien laaggradige + gelokaliseerde infectie zonder sepsis en gevoeligheid van zowel Enterobacter als MRSA. Behandeling in functie van klinische evolutie (10-14 dagen)
