4 voorwoord Crisis en gezondheid De economische crisis gaat niet ongemerkt aan ons voorbij; sommige landen worden harder getroffen dan anderen; dit geldt eveneens voor bedrijven binnen ons land. Crisis wordt doorgaans gekoppeld aan financiële middelen (euro s in Nederland), maar uit onderzoek blijkt dat de crisis ook gevolgen heeft voor de gezondheid. In The Lancet (oktober 2011) beschrijven Kentikelenis et al., dat in Griekenland de crisis en de daarmee samengaande toename van werkeloosheid, een grote toename in als slecht ervaren gezondheid van de (beroeps)bevolking tot gevolg heeft gehad en tot 25% meer zelfmoorden heeft geleid dan in Vooral gevoelige groepen, o.a. mensen met chronische aandoeningen en/of ouderen, worden het hardst getroffen. In tijden van crisis bezuinigen bedrijven op arbozorg en gaan ze over tot reorganisatie en inkrimping. Dit is vanuit financieel oogpunt wellicht een goede zet, maar vanuit gezondheidkundig oogpunt helemaal niet. Reorganiseren brengt stress teweeg en een verhoging van de werkdruk voor degenen die in het arbeidsproces blijven, met alle gevolgen van dien. Werknemers in zware omstandigheden verdienen steun om risico op uitval te verkleinen en arbeidsparticipatie te bevorderen. De monitoring en bewustwording van gezondheidsrisico s op de werplek, waarvoor ondersteuning en begeleiding door arboprofessionals cruciaal is, zouden in tijden van economische crisis juist niet het kind van de rekening moeten zijn, maar zoals gezegd, is helaas het tegendeel waar. Arbeidsparticipatie voor iedereen, is het motto van de overheid, maar dat vereist maatwerk en evaluatie van de maatregelen en van de begeleiding die wordt ingezet. Preventief Medisch Onderzoek (PMO) bijvoorbeeld, is een goede aanpak maar wordt nog niet door alle bedrijven toegepast en bovendien is onderzoek naar de effectiviteit van PMO voor arbeidsparticipatie nog schaars. De implementatie van verworven kennis en advies over de veiligheid en gezondheid op het werk is voor werknemers, werkgevers en overheid noodzakelijk om preventie van arbeidsgerelateerde aandoeningen te realiseren. Daarvoor is samenwerking en afstemming tussen onderzoek en praktijk noodzakelijk. Helaas is dit niet altijd aan de orde en nog minder in tijden van crisis. Praktijkgericht onderzoek is het afgelopen jaar een kernactiviteit van de afdeling geweest waarmee goede resultaten zijn behaald. Dit zal ook het komend jaar een uitdaging blijven. Wij gaan ervoor. 5 foreword Crisis and health The economic crisis is not passing us by unnoticed; some countries are being hit harder than others, and this also applies to companies in our country. Crisis is generally linked to financial resources (euros in the Netherlands), but research shows that the crisis is also having an impact on health. In The Lancet (October 2011), Kentikelenis et al., describe how in Greece the crisis and the accompanying increase in unemployment has resulted in a large increase in poor health as perceived by the (working) population and 25% more suicides than in Sensitive groups, such as people with chronic diseases and/or the elderly, are hit hardest. In times of crisis, companies economize on occupational health care as they turn to reorganization and cutbacks. While this may be a good move from a financial perspective, it is anything but good from a health point of view. Reorganization brings about stress and an increase in workload for those who remain employed, with all the associated consequences. Employees in tough conditions deserve support to reduce the risk of dropout and to promote participation in the labor force. The monitoring and awareness of health risks in the workplace, for which support and supervision by occupational health professionals is crucial, should not be the victim in times of economic crisis, but, as stated above, unfortunately the converse is true. Work participation for everyone is the motto of the government, but that requires customization and evaluation of the measures and of the support provided. Workers Health Surveillance (WHS), for example, is a good approach but is not yet applied by all companies, and, furthermore, research on the effectiveness of WHS for work participation is still sparse. The implementation of acquired knowledge and recommendations on safety and health at work is essential for employees, employers and the government in order to achieve prevention of work-related disorders. This requires cooperation and coordination between research and practice. Unfortunately this is not always on the agenda, and even less so at times of crisis. Practice-oriented research has been a core activity of the Institute in the past year, with good results achieved. This will continue to be a challenge in the year ahead. 6 organisatie Afdeling Coronel Instituut voor Arbeid en Gezondheid Het Coronel Instituut voor Arbeid en Gezondheid is een afdeling binnen het Academisch Medisch Centrum (AMC) te Amsterdam en ressorteert onder de Divisie Klinische Methoden en Public Health. Het instituut richt zich op vraagstukken op het gebied van arbeid en gezondheid. Het hoofd van de afdeling is prof. dr. Monique HW Frings- Dresen. De gezondheid van de werknemer of patiënt en de gezondheidszorg voor hen staan centraal. Er wordt gestreefd naar optimale preventie van beroepsziekten en bevordering van arbeidsparticipatie. Het Nederlands Centrum voor Beroepsziekten (NCvB), het Kenniscentrum Verzekeringsgeneeskunde AMC-UMCG-UWV-VUmc (KCVG), de Polikliniek voor Mens en Arbeid (PMA) en het Kenniscentrum Medische Keuringen in Arbeid (KMKA) zijn onderdelen van de afdeling. Het instituut neemt deel in de Netherlands School of Public and Occupational Health (NSPOH) en de AOWP (Academische OpleidingsWerkPlaats). The Coronel Institute of Occupational Health is a department of the Academic Medical Center (AMC) in Amsterdam and is part of the Division of Clinical Methods and Public Health. The Institute s research focuses on the field of occupational health. The department is headed by prof. dr. Monique HW Frings-Dresen. Its mission relates primarily to the health and healthcare of employees and patients. The aim is to optimize the prevention of occupational diseases and to promote participation in the labor force. The department includes the Netherlands Center for Occupational Diseases (NCvB), the Research Center for Insurance Medicine AMC-UMCG-UWV- VUmc (KCVG), the People and Work Outpatient Clinic (PMA) and the Expertise Center of Pre-employment Medical Examinations and Workers Health Surveillance (KMKA). The Institute participates in the Netherlands School of Public and Occupational Health (NSPOH) and the Academic Collaborative Training Center (AOWP). The World Health Organization (WHO) has designated the Coronel Institute of Occupational Health as a WHO Collaborating Center in Occupational Health. 8 onderzoek Ontstaan en preventie van arbeidsgerelateerde aandoeningen PI: prof. dr. Monique HW Frings-Dresen Development and Prevention of Occupational Diseases Doel onderzoekslijn De onderzoekslijn schenkt aandacht aan het ontstaan en de preventie van arbeidsgerelateerde aandoeningen i.e. beroepsziekten. Het onderzoek richt zich enerzijds op factoren in arbeid die (beroeps)ziekten veroorzaken en anderzijds op vraag stukken rond arbeidsre-integratie bij mensen met een (chronische) aan doening. Een aantal (chronische) ziekten/aandoeningen in relatie tot werk zijn aandachtspunt van studie, zoals aanhoudende vermoeidheid, klachten van het bewegingsapparaat, reuma, tumoren, en niet-aangeboren hersenletsel. Samen met de curatieve sector wordt onderzoek uitgevoerd op het gebied van arbeidsmogelijkheden tijdens het behandel proces van de patiënt, zoals bijvoorbeeld met het Gastro-Intestinaal Oncologisch Centrum Amsterdam (GIOCA), en reumatologie. Een aantal onderzoeksprojecten is gericht op de kwaliteit van verzekeringsgeneeskundig handelen, zoals bijvoorbeeld naar factoren die langdurige arbeidsongeschikt heid in stand houden, naar interventies die effectief zijn bij (zieke) werklozen met depressieve stoornissen en naar factoren die relevant zijn bij de beoordeling van jonggehandicapten (Wajongers) en voor begeleiding naar werk bij patiënten met niet-aangeboren hersenletsel. Wat is in 2011 bereikt? In 2011 is de tweede fase van het onderzoekprogramma van het Kenniscentrum Verzekeringsgeneeskunde AMC-UMCG- UWV-VUmc van start gegaan waarin als eerste stap met stakeholders is besproken wat relevante en op de praktijk toegespitste onderwerpen zijn. Het door de Stichting Instituut Gak PI: prof. dr. Monique HW Frings-Dresen Target This line of research addresses the etiology and prevention of work-related disorders, i.e. occupational diseases. The research focuses on work-related factors that cause occupational diseases as well as issues surrounding the occupational reintegration of people with chronic diseases. A number of chronic workrelated diseases/disorders are the subject of study. These include chronic fatigue, musculoskeletal disorders, rheumatism, tumors and acquired brain injuries. In cooperation with the curative sector research is being conducted on the possibilities of patients working during the treatment process. Examples include the Gastrointestinal Oncology Center Amsterdam (GIOCA) and rheumatology. A number of projects concern the quality of insurance medicine, such as the factors that perpetuate long-term occupational disability; finding effective interventions for people who are unemployed (due to illness) and suffer from depressive disorders; and factors that are relevant to the assessment of young people with disabilities (Wajong) and to helping people with acquired brain injuries find work. What was achieved in 2011? The second phase of the research program of the Research Center for Insurance Medicine AMC-UMCG-UWV-VUmc was launched in 2011, with the first step being to discuss relevant and practice-oriented issues with stakeholders. The research program Reintegration Improvement Study (RVO) funded by the Stichting Instituut Gak held its concluding symposium in 2011, with a small number of projects carrying on into Two doctorates were conferred, to Margot Joosen for her research on effective rehabilitation strategies for patients with chronic fatigue symptoms, and to Marie-Christine Plat for her research on the implementation of job-specific worker s health surveillance (WHS) for fire suppression personnel. These two doctoral programs were carried out under the responsibility of two PIs, Monique Frings-Dresen and Judith Sluiter, and attracted a lot of attention from the field. The feasibility of E-health intervention to help rheumatism patients retain their jobs and/or return to work was investigated in cooperation with the AMC s rheumatology department. A major implementation study to prevent musculoskeletal injuries in 8

9 research gefinancierde onderzoekprogramma Re-integratie Verbeter Onderzoek (RVO) heeft in 2011 het eindsymposium gehouden, een gering aantal projecten lopen nog door in het jaar Twee promoties hebben plaats gevonden, namelijk van Margot Joosen, gericht op effectieve re-integratie strategieën bij patiënten met aanhoudende vermoeidheidsklachten en van Marie-Christine Plat naar de implementatie van een beroepsspecifiek PMO voor repressief brandweerpersoneel. Deze twee promotietrajecten zijn onder de verantwoordelijkheid van twee PI s uitgevoerd, namelijk Monique Frings-Dresen en Judith Sluiter, en hadden veel belangstelling vanuit de praktijk. Samen met de afdeling reumatologie (AMC) is de haalbaarheid van een E-health interventie voor behoud en terugkeer naar werk voor reumapatiënten nagegaan. Ter preventie van klachten aan het bewegingsapparaat binnen de bouwsector is een groot implementatieonderzoek gestart in samen werking met EMGO+ (VUmc) en Arbouw. Op gebied van behoud en re-integratie naar werk voor patiënten gediagnosticeerd met kanker zijn een aantal subsidies geworven die in 2012 van start gaan; de samenwerking met GIOCA is gecontinueerd en heeft geresulteerd in een artikel in de BMJ Open over het aantal patiënten met werkgerelateerde klachten bij kanker. Met een subsidie van ZonMw is in samenwerking met professionals uit diverse disciplines en patiënten een richtlijn op gebied van beoordelen, behandelen en begeleiden van niet-aangeboren hersenletsel en arbeidsparticipatie gemaakt. Dit project wordt in 2012 gecontinueerd met een implementatie onderzoek van de richtlijn in de praktijk. Multidisciplinaire richtlijn Niet-Aangeboren Hersenletsel (NAH) en arbeidsparticipatie Door middel van literatuuronderzoek en consensus werd multidisciplinaire zorgafstemming ten aanzien van arbeids participatie geëffectueerd. Zorgverleners, die betrokken zijn bij NAH en arbeidsparticipatie, dienen over voldoende expertise te beschikken. Een kern van bedrijfsarts of verzekeringsarts in de arborol en revalidatiearts, bijgestaan door neuropsycholoog, arbeidsdeskundige (en eventueel) zorgcoördinator, is wenselijk. Samen dragen zij zorg voor eenduidigheid ten aanzien van informatieverstrekking en voor continuïteit in hulpverlening waarbij al vroeg aandacht besteed wordt aan arbeid en de visie van patiënt en mantelzorger worden meegenomen. De multidisciplinaire NAH-richtlijn voor beoordelen, behandelen en begeleiden (3B), biedt betrokken professionals, werkgevers en patiënten handvatten hoe arbeidsparticipatie bij NAH, vanaf het begin tot het moment van arbeidsparticipatie en daarna, kan worden vormgegeven. Wetenschappelijk onderzoek is noodzakelijk om dit proces verder te ontwikkelen en samenwerking tussen betrokken disciplines te intensiveren en te optimaliseren. Multidisciplinary guideline on Acquired Brain Injury and participation in the labor forcen Multidisciplinary healthcare coordination with regard to participation in the labor force was implemented through a review of the literature and consensus. Healthcare providers involved in acquired brain injury and participation in the labor force must have sufficient expertise. A core of an occupational physician or insurance physician of an occupational health service, assisted by a neuropsychologist, employment expert and healthcare coordinator (if necessary), is advisable. Together they are responsible for clarity with regard to the provision of information and for continuity of care, with attention for work and the views of the patient and informal carers taken into consideration at an early stage. The multidisciplinary acquired brain injury guideline for assessment, treatment and support serves as a guide for professionals, employers and patients in the interpretation of participation in the labor force for patients with acquired brain injuries, from the beginning up to the time of labor force participation and afterwards. Scientific research is needed in order to further develop this process and intensify and optimize cooperation between the disciplines concerned. the construction sector was launched in cooperation with EMGO+ (VUmc) and Arbouw. With regard to job retention and rehabilitation for patients diagnosed with cancer, a number of subsidies were acquired, which will come into effect in 2012; cooperation with GIOCA continued and resulted in an article in the BMJ Open on the number of patients with work-related symptoms and cancer. Thanks to subsidy from the Netherlands Organization for Health Research and Development (ZonMw), a guideline on the assessment, treatment and support of patients with acquired brain injuries and their participation in the labor force was developed in cooperation with professionals from various disciplines and patients. This project will be continued in 2012 with an implementation study of the guideline in practice. 9

10 onderzoek Kwaliteit van de zorg op het gebied van Arbeid en Gezondheid PI: prof. dr. Frank JH van Dijk Binnen het thema Kwaliteit van de zorg op het gebied van Arbeid en Gezondheid staan vier onderwerpen centraal: de mentale gezondheid, beroepsziekten, werkenden met een chronische aandoening en de verspreiding van wetenschappelijke kennis. Psychische problemen en aandoeningen hebben ongunstige gevolgen voor de werkende, maar mogelijk ook voor anderen zoals klanten, leerlingen en patiënten. Dit jaar verschenen drie publicaties van het project Mental Vitality at Work over een groot onderzoek bij verpleegkundigen en paramedisch personeel. Publicaties gaan over de eigenschappen van een nieuwe vragenlijst om het functioneren op het werk goed te kunnen meten. Steeds meer werkenden hebben een chronische aandoening. Kwalitatief onderzoek laat zien dat de bestaande begeleiding niet helpt bij de overgang van gedeeltelijke naar volledige werkhervatting bij werknemers met psychische problemen. Bij kankerpatiënten die weer aan het werk zijn komen vermoeidheid en depressie veel voor, samenhangend met weinig aanpassingen op het werk. Beide onderzoeken bieden aanknopingspunten voor de praktijk. Het blijkt dat de opvattingen van human resource managers en lijn managers over hoe werknemers te ondersteunen bij het behoud van hun werk, deels gelijk maar ook deels verschillend zijn. Dit is belangrijke informatie voor arbodeskundigen die nauw met hen samen werken. Een door ons zelf ontwikkelde empowerment training voor deze groep werknemers blijkt gelukkig succesvol: het zelfvertrouwen in het eigen kunnen neemt toe en de vermoeidheid wordt minder in vergelijking met een controlegroep. Quality of Care in Occupational Health PI: prof. dr. Frank JH van Dijk The four main topics within the Quality of Care in Occupational Health theme are: mental health, occupational diseases, employees with chronic diseases and the dissemination of scientific knowledge. Psychological problems and disorders have adverse effects on the affected employees themselves, and possibly also on others such as customers, schoolchildren and patients. This year, there were three publications by the Mental Vitality at Work project, a large study conducted among nurses and allied health professionals. The publications are about the qualities of a new questionnaire to measure performance at work. More and more employees have chronic diseases. Qualitative research shows that the existing support does not help in the transition from partial to full resumption of work among employees with psychological problems. Fatigue and depression, accompanied by few modifications at work, are 10

11 research Beroepsziekten komen nog steeds veel voor. Patiënten met een hersenaandoening door oplosmiddelen (chronic solvent encephalopathy) blijken veel problemen te hebben met depressie en angststoornissen, een constatering die kan leiden tot een betere begeleiding. Gunstig is een bevinding bij werkenden met klachten aan de arm, nek en/of schouder in een onderzoek door het Nederlands Centrum voor Beroepsziekten: zij blijken in één jaar tijd sterk te verbeteren in hun klachten. Meldingen van beroepsziekten door werkenden zelf is een nieuw onderzoeksthema. Een eerste artikel is verschenen. De vraag is wat er gebeurt met de duizenden wetenschappelijke artikelen over werken en gezondheid. Het belang van wetenschappelijke kennis voor het werk van verzekeringsartsen is met voorbeelden uitgewerkt in een artikel. Werknemers en leidinggevenden hebben, net als deskundigen, veel vragen over werken en gezondheid, maar kunnen vaak geen goede antwoorden vinden. Daarom is een online netwerk van deskundigen opgezet (ArboAntwoord) om vragen via het internet te beantwoorden. Dit netwerk wordt bijzonder gewaardeerd en biedt informatie van een duidelijk hogere kwaliteit vergeleken met antwoorden door zelf te zoeken op het internet. Het Coronel Instituut is internationaal actief, zoals voor de WHO. In Latijns Amerika en Spanje wordt gewerkt met onze publicatie in het Spaans over hoe te zoeken naar waardevolle informatie over werken en gezondheid op het internet. Cómo buscar la evidencia en las fuentes de internet? Salud Ocupacional Prof. F.J.H. (Frank) van Dijk Coronel Institute of Occupational Health Academic Medical Center Amsterdam The Netherlands common among cancer patients who return to work. Both studies are useful to improve health care practice. It turns out that the views of human resource managers and line managers on how to support employees in keeping their jobs are the same in some respects and different in others. This is important information for occupational experts who work closely with them. Fortunately, the empowerment training course we developed for this group of employees is proving successful, with an increase in self-confidence in their own ability and a decrease in fatigue compared to a control group. Occupational diseases are still common. Many patients with brain disorders caused by exposure to solvents (chronic solvent-induced encephalopathy) have been found to have problems with depression and anxiety disorders, a finding that can lead to better support. A study by the Netherlands Center for Occupational Diseases produced a favorable finding among employees with arm, neck and/or shoulder complaints: their symptoms improved significantly within one year. Reports of occupational diseases by employees themselves are a new theme of research. The first article has been published. The question is what happens to the thousands of scientific articles on work and health. The importance of scientific knowledge for the work of insurance company physicians was explained in an article through examples. Along with the experts, employees and managers have many questions about work and health, but are often unable to find good answers. Therefore an online network of experts (ArboAntwoord) has been set up to answer questions on the Internet. Judith K Sluiter Doel onderzoekslijn Kennisverwerving over de beoordeling van het werkvermogen (bv. tijdens medische keuringen) en het verbeteren van de prognose van functiespecifiek herstel na acute- of bij chronische gezondheidsproblematiek. Het goed kunnen (blijven) functioneren in werk is het streven. Onderzoekspopulaties: Werknemers in zware beroepen (bv. brandweer, ambulance, medisch specialisten (in opleiding), politiepersoneel, treinpersoneel, verpleegkundigen, leger, helikopterpiloten en helikoptercrew, topsporters, bouwpersoneel). Specifieke patiëntenpopulaties (bv. chronisch vermoeidheid, reuma/rsi, niet-aangeboren hersenletsel, stress/psychische klachten, (congenitale) hart- en vaatziekten). Wat is in 2011 bereikt? De gezondheid en werkvermogen van beroepen in de gezondheidszorg, en meer specifiek van ziekenhuispersoneel stonden centraal in De ontwikkeling van een functiespecifiek PMO voor medisch specialisten (in opleiding) is afgerond en de haalbaarheid van dit PMO is getest. Drie medisch specialismen van één UMC deden mee bij het testen. De arbodienst kon het opgestelde protocol goed uitvoeren en meer dan tweederde van de deelnemende artsen gaven aan te verwachten dat deelname aan het PMO en de verkregen adviezen hun gezondheid en werkfunctioneren positief kan beïnvloeden. Voor verpleegkundigen en paramedisch ziekenhuispersoneel is een psychische module voor PMO ontwikkeld en getest in een gerandomiseerd gecontroleerd onderzoek met drie onderzoeksarmen (een controlegroep en twee PMO strategieën). PMO met een bezoek aan de bedrijfsarts werd hierbij vergeleken met een volledig elektronisch PMO waarbij E-health interventies digitaal werden aangeboden. De menselijke, medische maat Workers Health Surveillance (WHS): work ability and recovery PI: dr. Judith K Sluiter Mission and research subjects To gain knowledge on work ability assessment (e.g. during medical examinations) and improve the prognoses for jobspecific recovery following acute or chronic health issues. The ultimate aim is to enable people to perform optimally at work. Study populations: Employees in high demands jobs (e.g. firefighters, ambulance workers, medical specialists/residents, police, train drivers, nurses, military personnel, helicopter pilots and crew, elite athletes and construction workers). Specific patient populations (e.g. chronic fatigue, rheumatoid arthritis/rsi, acquired brain injuries, stress/psychological disorders, (congenital) cardiovascular diseases). What was achieved in 2011? The health and work ability with regard to healthcare occupations, and specifically of hospital staff, were the focus in The development of a job-specific WHS for medical specialists and residents has been completed and the feasibility of this WHS has been tested. Verder is een evaluatieonderzoek afgerond naar een interventie gericht op het verminderen van de nekbelasting bij helikopterpersoneel. Het lopende cohortonderzoek onder medische studenten uit meerdere UMC s, dat in 2012 het derde jaar ingaat, heeft tot de eerste publicatie geleid (Van der Veer, Frings-Dresen en Sluiter, 2011, PLoS One) en veel aandacht gekregen in de landelijke pers. De ontwikkeling en implementatie van hulpmiddelen bij de medische beoordeling van werkvermogen staat centraal in twee projecten die door en voor verzekeringsgeneeskundigen worden uitgevoerd. Twee promotietrajecten zijn in 2011 succesvol afgerond: Margot Joosen (re-integratie van chronisch vermoeide werknemers met participatieproblemen) en Marie-Christine Plat (implementatie van beroepsspecifiek PMO voor repressief brandweerpersoneel) promoveerden. In 2011 zijn 11 nationale en 21 internationale wetenschappelijke publicaties verschenen. Op internationale congressen werd kennis verspreid via presentaties of posters. Maatschappelijk succes is dat zowel in de ambulance- als in de brandweersector via CAO of andere regelgeving vanaf 2011 is opgenomen dat de in deze onderzoekslijn ontwikkelde medische keuringen en preventief medisch onderzoek landelijk worden geïmplementeerd. Expertise is ingezet voor NWO en ZonMw. Met het Kenniscentrum Medische Keuringen in Arbeid zijn diverse onderwijs- en adviesactiviteiten ondernomen. Twee nieuwe ZonMw projecten starten in 2012: een nieuwe maat voor werkfunctioneren wordt ontwikkeld in de komende jaren en samen met de NVAB is een project gestart waarin het preventieconsult in de bedrijfsgezondheidszorg wordt uitgetest. Ontwikkeling, implementatie en evaluatie van medische keuringen (aanstellingskeuringen, verplichte medische keuringen en preventief medisch onderzoek) wordt in deze Onderzoekslijn in samenwerking met praktijkpartijen vormgegeven. Een kenniscentrum op het gebied van medische keuringen wordt geleid samen met de NVAB, het Kennis centrum Medische Keuringen in Arbeid (KMKA), University Medical Center took part in the testing. The cohort study among medical students from multiple UMCs, which enters its third year in 2012, has led to its first publication (Van der Veer, Frings-Dresen and Sluiter, 2011, PLoS One) and received a lot of attention from the national media. The development and implementation of tools used in the medical assessment of work ability are the focus of two projects being carried out by and on behalf of insurance company physicians. Als mogelijke gevoeligheidsgenen worden een aantal polymorphismen onderzocht in genen die betrokken zijn bij de vorming van de huidbarrière en bij het reguleren van ontstekingsprocessen. De studies richten zich met name op de functionele gevolgen van de genetische variaties in het filaggrine gen (FLG). FLG mutaties leiden tot een minder goed functionerende huidbarrière, waardoor de huid meer doorlaatbaar wordt voor stoffen die sensitisatie en ontstekingen in de huid kunnen veroorzaken. Deze mutaties zijn een belangrijke factor in de predispositie voor atopische dermatitis (AD). Individuen met een (geschiedenis van) AD hebben een grotere kans op het ontwikkelen van CE. Echter, de afzonderlijke bijdragen van de FLG mutaties en AD voor het verkrijgen van CE zijn niet bekend. Om te bestuderen in hoeverre FLG mutaties en AD bijdragen tot het ontwikkelen van beroepsmatig CE werden twee onderzoeken uigevoerd: een prospectief onderzoek uitgevoerd bij leerling-verpleegkundigen en een case-control studie bij patiënten met chronisch contacteczeem. De functionele gevolgen van FLG mutaties en AD werden onderzocht in een experimentele irritatiestudie. 15 research Individual susceptibility to (occupational) skin disorders PI: dr. Sanja Kezic This line of research focuses on the significance of genetic variations in the development and prognosis of occupational contact eczema (CE). A number of polymorphisms in genes which are involved in the formation of the skin barrier and in the regulation of inflammatory processes are being investigated as possible sensitivity genes. The studies focus primarily on the functional consequences of genetic variations in the filaggrin gene (FLG). FLG mutations lead to an improperly functioning skin barrier, as a result of which the skin becomes more permeable to substances that can cause sensitization and inflammation in the skin. These mutations are an important factor in the predisposition for atopic dermatitis (AD). Individuals with a history of AD have a greater chance of developing CE. However, the individual contributions of the FLG mutations and AD in the development of CE are not known. To investigate the extent to which FLG mutations and AD contribute to the development of occupational CD, two studies are being conducted: a prospective study among student nurses and a case-control study among patients with chronic contact eczema. The functional consequences of FLG mutations and AD were investigated in an experimental irritation study. Healthy volunteers and AD patients with and without FLG mutations were exposed to water and to a model irritant (sodium lauryl sulfate). The irritation response was observed by measuring the skin barrier, skin erythema and the level of various inflammatory mediators and breakdown products. In addition, the morphological changes in the epidermis were studied by means of confocal microscopy (Vivascope, MAVIG). Paul BA Smits In 2011 heeft het Coronel Instituut voor Arbeid en Gezondheid meer dan 200 studenten Geneeskunde in de juniorco- en in de coschappen getraind om scherper te letten op de rol van werk bij het ontstaan van ziekten èn de zorg voor zieken ook te richten op deelnemen aan werk. Dit is gelukt dankzij de goede samenwerking met de Arbodienst van het AMC, ArboUnie, Achmea Vitale en HumanCapitalCare Arbozorg, en anderen zoals sportartsen en zelfstandig bedrijfsartsen. Ook het 2e jaars keuzeonderwijs Topsport, dans en muziek als werk is weer goed beoordeeld. Het Coronel Instituut voor Arbeid en Gezondheid is ook actief in de AMC opleiding Medische Informatiekunde en in de opleiding, bij- en nascholing van bedrijfs- en verzekeringsartsen bij de Netherlands School of Public & Occupational Health (NSPOH). Dit laatste gebeurt in de Academische OpleidingsWerkPlaats (AOWP) voor Arbeid, Maatschappij en Gezondheid. In 2011 is een driejaarlijkse Topclass Occupational Health onder verantwoordelijkheid van prof. dr. Carel Hulshof gestart. Binnen de opleiding voor verzekeringsartsen is de samenwerking met het Kennis centrum Verzekeringsgeneeskunde AMC-UMCG-UWV-VUmc versterkt. Daarnaast verzorgt het Coronel Instituut onderwijs bij in company opleidingen, onder andere met het Nederlands Centrum voor Beroepsziekten. Met KLM Health Services is een workshopcyclus over het signaleren van beroepsziekten gegeven. De staf onderwijs zorgt voor ontwikkeling, coördinatie en kwaliteitsborging. 18 monitoring en signalering Monitoring en signalering van beroepsziekten Het Nederlands Centrum voor Beroepsziekten (NCvB) registreert in opdracht van het ministerie van Sociale Zaken en Werkgelegenheid het vóórkomen van beroepsziekten in Nederland. Bedrijfsartsen en verzekeringsartsen in de arborol zijn wettelijk verplicht deze te melden aan het NCvB. Het doel van deze beroepsziekteregistratie is het stimuleren van preventie van werkgerelateerde aandoeningen, individueel én collectief. Immers elke beroepsziekte is te vóórkomen. In het verslagjaar 2011 is met een vragenlijstonderzoek extra aandacht besteed aan het verkrijgen van inzicht in de factoren die het melden van beroepsziekten beïnvloeden. Er lijken nu meer barrières te bestaan dan tien jaar geleden voor een werkende om een mogelijk werkgerelateerd gezondheidsprobleem voor te leggen aan een bedrijfsarts. De meldende bedrijfsartsen blijken vooral intrinsiek gemotiveerd door hun professionele taakopvatting. In 2011 zijn meldingen van beroepsziekten geregistreerd. Het merendeel van alle meldingen over 2011 komt uit drie categorieën beroepsziekten, namelijk: gehooraandoeningen (40%), aandoeningen van het houding- en bewegingsapparaat (34%) en psychische aandoeningen (19%). In tabel 1 staan de tien meest gemelde beroepsziekten van In totaal melden 174 bedrijfsartsen via het Peilstation Intensief Melden de door hen gediagnosticeerde beroepsziekten. Daarnaast wordt extra informatie gevraagd over de aard en omvang van hun werknemerspopulatie. In 2011 zijn via dit peilstation 1472 meldingen van beroepsziekten geregistreerd, hetgeen een incidentie betekent van 302 (95% betrouwbaarheidsinterval (BI): ) beroepsziekten per werknemersjaren. 19 monitoring and notification Monitoring and notification of occupational diseases The Netherlands Center for Occupational Diseases (NCOD) registers occupational diseases in the Netherlands for the Ministry of Social Affairs and Employment. Occupational physicians of occupational health services and insurance physicians are required by law to report these diseases to the NCOD. The purpose of registering these occupational diseases is to stimulate the prevention of work-related disorders, individually and collectively. After all, every occupational disease can be prevented. During 2011, a questionnaire was used to devote extra attention to obtaining insight into the factors that influence the notification of occupational diseases. There appear to be more barriers now than there were ten years ago for employees to put possible occupational health problems to an occupational physician.
