6 1 Wetenschapsbeleid In 2013 heeft de wetenschapscommissie in praktische zin het wetenschapsbeleid verder vormgegeven. Ook werd het document Beleid Wetenschappelijk Onderzoek Haga Ziekenhuis verder geoptimaliseerd. Hierin staat beschreven hoe de ziekenhuisorganisatie samen met de onderzoekers vorm geven aan klinisch wetenschappelijk onderzoek. Hopelijk kan de tweede versie van het beleidsstuk in 2014 door alle partijen worden geaccodeerd. Ondertussen is uitvoering gegeven aan het beleid zoals geformuleerd in de eerste versie van dit beleidsstuk. In 2013 werd de administratieve afhandeling van protocollen verder gestroomlijnd zodat onder andere aan de verplichtingen, die door de landelijk Richtlijn Externe Toetsing (RET) bij de ziekenhuisorganisaties kwam te liggen, kon worden voldaan. T.C.A.M. van Woerkom 2 Wetenschapsbureau Klinisch wetenschappelijk onderzoek is een belangrijke activiteit in een groot opleidingsziekenhuis zoals het HagaZiekenhuis. Dit onderzoek dient dan ook goed ingebed te zijn in de organisatiestructuur om te kunnen voldoen aan alle voorwaarden voor veilig en integer onderzoek. Hiermee wordt dan ook toegezien op uitvoering volgens wet- en regelgeving. In 2011 is dit door de Raad van Bestuur HagaZiekenhuis onderkend. Er is een start gemaakt met een Wetenschapsbureau om de verdere professionalisering vorm te geven. De Wetenschapscommissie van het HagaZiekenhuis heeft in 2012 mede aan de basis van deze ontwikkeling gestaan. In 2013 kon het bureau zich verder ontwikkelen en vorm geven aan haar taken. CURSUSSEN Ook in 2013 werd aandacht besteed aan scholing opdat het klinisch wetenschappelijk onderzoek nog veiliger en effectiever kan plaatsvinden. Voor zowel interne medewerkers maar ook voor geinteresseerden van buiten het ziekenhuis werden GCP-cursussen georganiseerd. 3 Behandelde onderzoeken In 2013 zijn er meer onderzoeken aangeboden dan in het voorafgaande jaar. In het af gelopen jaar werden alle onderzoekers benaderd van studies die bij het wetenschapsbureau in 2011 en 2012 waren ingebracht en die medio 2013 nog niet van start waren gegaan. Aan een aantal kon hulp gegeven worden om de problemen rond de organisatie van de studie op te lossen. Van sommige studies werd in overleg met de studiecoordinator duidelijk dat het nooit tot een implementatie zou komen. De procedure rond deze onderzoeken werd gestaakt en indien noodzakelijk werd de METC daarvan in kennis gesteld. 8 Tweeenveertig van de ingediende protocollen betrof investigator initiated onderzoek. Hiervan werden er twee negatief beoordeeld door de METCZWH. Tien van de Investigator Initiated protocollen waren WMO-plichtig. Bij de dertig overgebleven protocollen betrof het niet WMO-plichtig onderzoek. Van vijfentwintig protocollen waren op nog niet alle administratieve handelingen verricht zodat deze niet voor definitieve ondertekening aan de Raad van Bestuur konden worden aangeboden. Opvallend was de vaak lange wachttijd tot alle documenten aanwezig waren. Wanneer een onderzoek binnen de reguliere zorg valt, wordt een nulbegroting ingediend. Veel van de in 2013 gestarte onderzoeken hebben zo n nulbegroting. Na 1 juli 2012 moesten de industriegesponsorde onderzoeken rekening houden met een extra bedrag voor het wetenschapsfonds (8%) en de overhead (8%), in totaal 16% boven op het sponsorbedrag in relatie tot het betreffende onderzoek. Slechts bij één onderzoek, geïnitieerd in 2013, werd er ook daadwerkelijk een storting in het wetenschapsfonds verricht. WETENSCHAPSFONDS Bij veel industriegesponsorde onderzoeken ontvangt de onderzoeker een vergoeding per geïncludeerde patiënt. Conform het wetenschapsbeleid HagaZiekenhuis wordt bij industriegesponsorde onderzoeken waarvan de onderhandelingen plaatsvinden na 1 juli 2012, de 8% voor het wetenschapsfonds in de onderhandeling meegenomen. In 2013 vond de eerste storting in het wetenschapsfonds plaats (de IDEA-studie van de Kindergeneeskunde). Ook de Raad van Bestuur deed de door haar beloofde bijdrage aan het Wetenschapsfonds in Er is een batig saldo van ruim 200K. 10 4 Haga Wetenschapsdag 2013 De Haga Wetenschapsdag vond op 21 november 2013 plaats in het nieuwe Auditorium van het HagaZiekenhuis. Op de wetenschapsdag waren 170 deelnemers aanwezig. Dit jaar werden er 71 abstracts ingediend. Er vond in 2013 voor het eerst een selectie plaats op basis van de inhoud van het ingstuurde abstract. Uit de inzendingen werden door de wetenschapscommissie 60 abstracts uitgekozen voor een posterpresentatie. Hieruit werden 6 abstract gekozen voor een aparte presentatie tijdens de poster walk. Tevens werden 6 abstract gekozen voor een orale presentatie. Dr. H.P. (Harm) Sleeboom hield een voordracht getiteld Moderne behandeling van osteoporose. [Vlnr.] D. (Dorothea) Evers, AIOS Hematologie, R.J. (Rolf ) Verheul, AIOS Klinische Chemie, en dr. J.W.S. (Jos) Merkus, chirurg zijn de gelukkige prijswinnaars op de Haga Wetenschapsdag van 21 november 2013 Voorafgaand aan deze wetenschapsdag waren jonge onderzoekers uitgenodigd voor een scholingsprogramma over het presenteren van (klinisch wetenschappelijk) onderzoek. yde posterprijs is dit jaar gegaan naar drs Rolf Verheul van het Klinisch Chemisch en Hematologisch Laboratorium voor de poster Leeftijdsafhankelijke afkapwaarden voor de D-Dimeer test bij verdenking op een veneuze-embolie: zo klaar als een klontje. yde afdelingsprijs is dit jaar gegaan naar de afdeling Heelkunde. yde dr. Chanfleury van IJsselsteinpsrijs is dit jaar gegaan naar Mevr. Drs. Dorothea Evers van de afdeling Hematologie voor haar artikel The efficiency of therapeutic erythrocytapheresis: a mathematical tool for predicting response in hereditary hemochromatosis, polycythemia vera and secondary erythrocytosis. 5 Vooruitblik 2014 Voor 2014 zal het een uitdaging zijn om alle partijen die betrokken zijn bij wetenschappelijk onderzoek het beleidsplan Wetenschappelijk onderzoek HagaZiekenhuis te laten accorderen. De verdere professionalisering van het wetenschapsbureau HagaZiekenhuis staat op het programma. De nadruk zal liggen op: yaanschaf datamanagement systeem yinterne monitoring van studies. 11 6 Jaaroverzicht per afdeling Publicaties en abstracts uit 2013 waaraan medewerkers van het HagaZiekenhuis hebben bijgedragen per afdeling. Sommige items komen bij meer dan één afdeling voor. ANESTHESIOLOGIE Publicaties yjungschläger H: Postoperatieve misselijkheid en braken. Tijdschr. voor Anesthesiologie 2013: 13; APOTHEEK Speciale projecten yfarma-xl: binnen het samenwerkingsverband van Haga, RdGG, SFG, Vlietlandziekenhuis en IJssellandziekenhuis worden farmaceutische zorgpaden geïmplementeerd om de zorg naar een hoger kwaliteits- en doelmatigheidsniveau te brengen. Deze behandelprotocollen zijn gebaseerd op de meest recente kennis uit de literatuur, landelijke richtlijnen en eigen ervaringen. Door structureel parameters te meten en te registreren wordt de zorg evalueerbaar. De individuele behandeling van een patiënt kan direct worden bijgesteld op basis van deze parameters. Gelijktijdig kan uitkomstenonderzoek binnen patiëntgroepen leiden tot bijstelling van de zorgpaden. yhet HAGA project Een betere medicatieoverdracht door scholing, samenwerking en procesoptimalisatie beoogt de medicatieoverdracht in het HagaZiekenhuis in de spoedsetting meetbaar naar een hoger niveau te tillen. Eind 2012 is een nulmeting uitgevoerd, waarbij duidelijk werd dat vaak onbewuste discrepanties met de thuismedicatie optreden. Vervolgens is in 2013 het medicatieoverdrachtsproces geoptimaliseerd door o.a. informatie over thuismedicatie beter vindbaar en toepasbaar te maken en scholingen te geven. Een nameting heeft in november 2013 plaatsgevonden en wordt op dit moment verwerkt. Onderzoekslijnen 1. De onderzoekslijn Cystic Fibrosis met het centrum voor volwassen CF-patiënten van het HagaZiekenhuis is de langstlopende onderzoekslijn binnen de AHZ. Deze onderzoekslijn heeft naast registratieonderzoek al 4 proefschriften van ziekenhuisapothekers opgeleverd. Momenteel promoveert Annelies van Velzen binnen deze lijn. Primair verantwoordelijk: J Uges, ziekenhuisapotheker. 2. Medicatieveiligheid rondom IC opnamen: onderzoek naar medicatieverificatie rondom IC opnamen en het optimaliseren van farmacotherapie door aanwezigheid van een apotheker tijdens de IC visite. Binnen deze onderzoekslijn wordt samengewerkt met het Erasmus MC. Promovenda en primair verantwoordelijk voor deze lijn: mw BE Bosma, ziekenhuisapotheker. 3. Klinische farmaco-epidemiologie: onderzoek naar de veiligheid en effectiviteit van geneesmiddelen mbv van gegevens die routinematig worden vastgelegd.
