5 Ribosome The ribosome is a large and complex molecular machine, that serves as the primary site of biological protein synthesis (translation). Ribosomes link amino acids together in the order specified by messenger RNA (mrna) molecules. Ribosomes consist of two major subunits the small ribosomal subunit reads the mrna, while the large subunit joins amino acids to form a polypeptide chain Prokaryotes have 70S ribosomes, each consisting of a small (30S) and a large (50S) subunit. Their small subunit has a 16S RNA subunit bound to 21 proteins. The large subunit is composed of a 5S RNA subunit, a 23S RNA subunit and 31 proteins

8 Werkingsspectrum macroliden Gevoelig Gram + streptococcen en staphylococcen Gram - Bijv. Neisseria gonorrhoeae, Moraxella catarrhalis, Mycoplasma pneumoniae, Ureaplasma urealyticum, Chlamydia trachomatis, Mycobacterium avium complex en Campylobacter spp. Ongevoelig Enterobacteriaceae en pseudomonas ssp.

9 Werkingsspectrum lincomycinen Het antimicrobiële spectrum van clindamycine lijkt op dat van de macroliden, maar is iets smaller (C. jejuni en Legionella niet gevoelig) Wel gevoelig zijn anaëroben (Clostridium, B. fragilis).

10 Indicaties macroliden/clinda Infecties door micro-organismen die penicillinen resistent zijn en infecties bij patiënten die overgevoelig zijn voor penicilline. Infecties van de luchtwegen en longen (Voor Mycoplasma pneumoniae, Campylobacter, Legionella en Chlamydia behoren de macroliden tot de weinige middelen waarvoor deze micro-organismen gevoelig zijn) PID door Chlamydia trachomatis (azitromycine 1000 mg po eenmalig) Infecties met Mycobacterium avium intracellulare bij hiv-patiënten 2 e keus bij Toxoplasma bij hiv-patienten Helicobacter pylori eradicatie Anaërobe infecties (clindamycine) Osteomyelitis door staphylococcen (clindamycine)

13 Target modification Ribosomal methylation (first decribed, now most prevalent) Acquisition of erm (erythromycin ribosome methylation) gene, encoding a methyltransferase, which, methylates the N(6) position of adenine 2058 in 23S rrna Strains harboring the A2058 methylation do not bind macrolides, thus causing resistance Monomethylation confers a high level of resistance to lincosamides and streptogramins and a lower level to macrolides Dimethylation confers high levels of resistance to the three classes of drugs (macrolide-lincosamide-streptogramin/mlsb phenotype of crossresistance)

15 Target mutation Mutation of 23S rrna, with substitution of adenine 2058 by a guanine being the most common one, have also been described in bacterial pathogens (mainly found in Helicobacter pylori, Mycoplasma spp, and Mycobacterium spp.) Mutations in the ribosomal proteins L4 and L22 have also been recently associated with resistance to macrolides in clinical strains of streptococci

16 Efflux In Gram-pos bacteria, the expression of efflux pumps confering resistance to macrolides is inducible macrolides are inducer clindamycin is neither inducer nor substrate Two main type of pumps with narrow spectrum have been described: msra in staphylococci mefa and mefe described in several species of streptococci including S. pneumoniae and S. pyogenes, as well as enterococci mefa: located on a conjugative transposon -> can easily spread between bacteria

17 Antibiotic inactivation Confers resistance to structurally related antibiotics only (affects macrolides and not lincosamides or streptogramins) Phosphorylases and esterases (mainly reported in Enterobacteriaeae) Few strains of phosphotransferases producing S. aureus have already been reported (mechanism may become more significant in the future)

18 Remarks Clarithromycin Therapy with clarithromycin has been shown to increase macrolide resistance in oropharyngeal flora Clarithromycin resistance in Helicobacter pylori appears to be increasing and is often related to previous use of macrolides Resistance is due to a point mutation in the 23S RNA Azithromycin Immunomodulatory agent -> has been given for prolonged time to CF patients Increased resistance of S. aureus and H. influenzae
