5 Inleiding Introduction Dit jaarverslag geeft een overzicht van de activiteiten van het NCvB in De activiteiten van het NCvB richten zich op kennis en zorg met betrekking tot beroepsziekten. Kennisverzameling vindt plaats met behulp van diverse instrumenten zoals de registratie van beroepsziekten, peilstations voor beroepsziekten, literatuursearches en expertnetwerken. Kennisverspreiding vindt onder andere plaats via scholing en nascholing, de helpdesk, de website en publicaties. In dit jaarverslag vindt u een overzicht van de instrumenten die het NCvB inzet voor verzameling, ordening, analyse en verspreiding van kennis over beroepsziekten. De beeldpagina s van dit jaarverslag besteden aandacht aan de communicatie over beroepsziekten aan het algemene publiek via de media. De alert- of signaleringsfunctie is het centrale concept in het kennismanagement van het NCvB. Figuur 1 geeft deze functie in schema weer. Gathering knowledge takes place with the assistance of several instruments such as the registration of occupational diseases, surveillance projects for occupational diseases, literature searches and expert networks. Dissemination of knowledge takes place, among other means, through education and continuing education, the helpdesk, the website and publications. In this annual report you will find an overview of the instruments the NCvB is introducing for collecting, planning, analysis and dissemination of knowledge about occupational diseases. The illustrated pages of this annual report focus the attention on the communication about occupational diseases to the general public by the media. The Alert or warning signal function is the central concept in the NCvB s knowledge management. Figure 1 shows this function schematically. 6 occupational diseases. In knowledge gathering, planning, analysis and dissemination the NCvB is continuously searching for elements of knowledge that have this significance. In the Alert Report on Occupational Diseases attention is first of all requested for the most important Alerts. A good national and international network is an important requirement for functioning as a knowledge meeting point. The NCvB pays a lot of attention to this, as will become evident from the appendixes of this report. In The Netherlands, there are gaps in care and knowledge with respect to complex occupational medical problems, in this case occupational diseases. Other western countries have specialised (outpatient) clinics for this. In The Netherlands such an infrastructure is missing. The NCvB in cooperation with clinical departments of Amsterdam s Academic Medical Center (AMC) developed a number of clinical occupational medical care arrangements, such as the Solvent Team and the Expertise Center for Hearing and Work. The NCvB is looking to expand these second and third line care arrangements as well as play a leading role in a future clinical occupational medical infrastructure in our country. niselementen die deze betekenis hebben. In Nederland is sprake van lacunes in zorg en kennis ten aanzien van complexe arbeidsgeneeskundige problematiek, i.c. beroepsziekten. In andere westerse landen bestaan hiervoor gespecialiseerde (poli)klinieken. In Nederland ontbreekt een dergelijke infrastructuur. Het NCvB heeft samen met klinische afdelingen in het AMC een aantal klinisch arbeidsgeneeskundige zorgarrangementen ontwikkeld, zoals het Solvent Team en het expertisecentrum Gehoor en Arbeid. Het NCvB heeft de ambitie deze tweede- en derdelijns zorgarrangementen uit te breiden en een leidende rol te spelen in een toekomstige klinisch arbeidsgeneeskundige infrastructuur in ons land. 7 Informatie verzamelen en ordenen Information gathering and planning Registratie van beroepsziekten Om inzicht te krijgen in de ontwikkeling en de spreiding van beroepsziekten is een goede registratie essentieel. Het Nationale Melding- en Registratiesysteem is het belangrijkste en omvangrijkste registratiesysteem binnen het NCvB. Dit systeem geeft o.a. informatie over het vóórkomen en de verspreiding van beroepsziekten in ons land, en trends hierin. In tabel 1 is een top 10 van de geaccepteerde meldingen beroepsziekten in 2003 te zien. Met dit registratiesysteem is echter niet alle benodigde informatie te verzamelen. Daarom is een aantal peilstations opgezet die extra informatie opleveren. Nationale melding- en registratiesysteem In dit registratiesysteem worden de meldingen van beroepsziekten door (bedrijfs)artsen verzameld. Sinds 1999 is de melding een wettelijke verplichting voor Arbo-diensten. Omdat er nog steeds sprake is van een behoorlijke onderreportage is er het afgelopen jaar extra energie gestoken in het bezoeken van een aantal grote landelijke Arbo-diensten. Tijdens deze bezoeken is aandacht besteed aan het bevorderen en vergemakkelijken van het melden en het geven van voorlichting over het nut van melden aan bedrijfsartsen. In 2003 zijn 5973 meldingen van een Registration of occupational diseases In order to gain insight in the development and distribution of occupational diseases good registration is essential. The National Notification and Registration System is the most important and extensive registration system within the NCvB. This system gives, among other things, information on the prevention and the dissemination of occupational diseases in our country, and the relevant trends in this field. In Table 1 a top 10 of the accepted occupational diseases in 2003 can be seen. However, with this registration system not all necessary information can be gathered. Therefore a number of surveillance projects have been set up that provide additional information. National notification and registration system In this registration system the notifications of occupational diseases by (occupational) physicians are collected. Since 1999 notification is a legal obligation for Occupational Health Services (Arbo-diensten). As there still is a considerable lack of notification, a lot of energy has been invested in visiting a number of large Occupational Health Services in the past year. In 2003 the NCvB has accepted a total of 5973 notifications of occupational diseases. This is a slight increase with regard to last year. Table 2 shows an overview of the number of accepted notifications the NCvB has received in the past three years. Approximately 60% of all notifications are filed electronically. In 2004 more attention will be given to the possibility of notification via the Occupational Health Services own system. Making the notification via Internet more userfriendly is also being worked on. Both of these activities will hopefully lead to a further increase in electronic notification. In 2003 notification information on the year 2001 has been filed for the Eurostat project European Occupational Diseases Statistics (EODS). The project s objective is to improve agreements on criteria and coding of occupational diseases. Ultimately this will provide better comparable statistics of occupational disease in European countries. beroepsziekte door het NCvB geaccepteerd. Ten opzichte van vorig jaar betekent dit een lichte stijging. In tabel 2 wordt een overzicht gegeven van het aantal geaccepteerde meldingen dat het NCvB in de afgelopen drie jaar ontvangen heeft. Ongeveer 60% van alle meldingen wordt elektronisch aangeleverd. In 2004 wordt meer aandacht gevraagd voor de mogelijkheid om via het eigen systeem van de Arbo-dienst te melden. Ook wordt gewerkt aan het gebruikersvriendelijker maken van het melden via Internet. Deze twee activiteiten zullen hopelijk leiden tot een verdere toename van het elektronisch melden. In 2003 zijn er over het meldingsjaar 2001 gegevens aangeleverd voor het Eurostat project European Occupational Diseases Statistics (EODS). Dit is een project wat tot doel heeft om afspraken over criteria en codering van beroepsziekten te verbeteren. Dit moet uiteindelijk beter vergelijkbare beroepsziektestatistieken van Europese landen opleveren. The NCvB has drawn up a number of occupational diseases registration guidelines for notification of occupational diseases. In 2003 a Steering Committee on Guidelines was established. This steering committee will revise the existing guidelines and set up new registration guidelines. As a number of occupational diseases were added to the European list of occupational diseases in 2003, they will determine for which of these disorders registration guidelines have to be developed. Voor het melden van beroepsziekten zijn door het NCvB van een aantal beroepsziekten registratie richtlijnen opgesteld. In 2003 is een Stuurgroep Richtlijnen ingesteld. Deze stuurgroep gaat bestaande richtlijnen herzien en nieuwe registratie richtlijnen opstellen. Omdat er in 2003 een aantal beroepsziekten is toegevoegd aan de Europese lijst van beroepsziekten, wordt bezien voor welke van deze aandoeningen registratie richtlijnen ontwikkeld moeten worden. ADS (Occupational Dermatoses Surveillance) The occupational physician does not often see employees with occupational skin disorders. This results in an important part of skin disorders not being reported to the National Notification and Registration System. In order to still obtain relevant information on skin disorders a surveillance project was established in cooperation with the NECOD ADS (ArbeidsDermatosen Surveillance) Werknemers met beroepsgebonden huidaandoeningen worden vaak niet door de bedrijfsarts gezien. Dit betekent dat een belangrijk deel van huidaandoeningen niet gemeld wordt in het Nationale melding- en registratiesysteem. Om toch relevante informatie over huidaandoeningen te krijgen is in samenwerking met het NECOD (Nederlands Kenniscentrum ArbeidsDermatosen) een peilstation opge- Nederlands Centrum voor Beroepsziekten Jaarverslag 03 Netherlands Center for Occupational Diseases Annual report 03 7

9 zet waar 23 dermatologen beroepsgebonden huidaandoeningen melden. In 2003 zijn door deze dermatologen meer dan 600 meldingen gedaan. (Netherlands Expert Center for Occupational Dermatoses) to which 23 dermatologists report occupational skin disorders. In 2003 these dermatologists reported more than 600 cases. PAL (Peilstation Arbeidsgebonden Longaandoeningen) Analoog aan het ADS peilstation is er een peilstation voor beroepsgebonden longaandoeningen opgezet. Dit project is het resultaat van een samenwerking met het NKAL (Nederlands Kenniscentrum Arbeid en Longaandoeningen Opgelucht Werken). Momenteel zijn er 17 maatschappen van longartsen die maandelijks hun meldingen van beroepsgebonden longaandoeningen melden. Omdat het aantal meldingen wat tegenvalt, wordt aan dit project in 2004 extra aandacht besteed. PAL (Surveillance Project for Work-related Lung Disorders) Analogous to the ADS surveillance project a surveillance project for work-related lung disorders was established. This project is the result of cooperation with NKAL (Knowledge Center for Work-related Lung Diseases). At the moment there are 17 partnerships of lung specialists who make monthly notifications of occupational lung disorders. Because the number of notifications is somewhat disappointing, this project will receive additional attention in PIM (Peilstation Intensieve Melding) Een aantal gemotiveerde bedrijfsartsen uit het Nationale melding- en registratiesysteem is in 2002 benaderd om deel te nemen aan het PIM project. Van de 44 artsen die deelnemen aan dit project is de verzorgde populatie bekend. Op deze wijze kan een beter beeld verkregen worden van de incidentie van beroepsziekten binnen verschillende sectoren. Door middel van een uitgebreid vragenlijstonderzoek bij de gemelde werknemers wordt gevraagd naar de sociale en economische consequenties van beroepsziekten. De gegevensverzameling voor dit onderzoek zal doorlopen tot en met Peilstation Politie Dit project wordt in opdracht van het Ministerie van Binnenlandse Zaken in samenwerking met de LOPOL (werkgroep van bedrijfsartsen werkzaam voor de politie) uitgevoerd. Twaalf bedrijfsartsen verdeeld over 3 Arbo-diensten zijn 1 april 2002 begonnen met het melden van beroepsziekten bij politiepersoneel. Inmiddels zijn er 66 meldingen binnengekomen. Bij deze meldingen wordt ook de verzuimduur geregistreerd om extra inzicht te krijgen in de gevolgen van beroepsziekten bij de politie. De gegevensverzameling voor dit project zal doorlopen tot april Werkgroepen Werkgroepen rondom een bepaald thema op het gebied van beroepsziekten vormen voor het NCvB een belangrijke bron van informatie en hebben daarnaast ook een duidelijke klankbordfunctie. Binnen de werkgroepen treffen het veld van de bedrijfsgezondheidszorg, de kliniek en de wetenschap elkaar. PIM (Surveillance Project for Intensive Notification) In 2002 a number of motivated occupational physicians from the National Notification and Registration System were approached to take part in the PIM project. The population cared for by the 44 participating physicians in this project is known. This way a better picture can be obtained of the incidence of occupational diseases within the different sectors. The socio-economic consequences of occupational diseases are assessed using an extensive study of the reported employees with questionnaires. The gathering of data for this study will continue through Surveillance Project for Police This project by order of the Ministry of the Interior was carried out in cooperation with LOPOL (working group of occupational physicians employed by the police). Twelve occupational physicians divided across 3 Occupational Health Services started on 1 April 2002 with notification of occupational diseases amongst police personnel. In the mean time 66 notifications have been filed. In these notifications the duration of the absence is also registered in order to obtain additional insight into the consequences of occupational diseases for the police force. The gathering of information for this project will continue until April Working groups Working groups focussing on a specific theme in the field of occupational diseases form an important source of information for the NCvB and besides that also provide a clearly evident sounding board function. Within the working groups the fields of occupational healthcare, clinical practice and science come together. The Working group on Work-related Infectious diseases (WIZA) only has a short history in which they were dealing with several actual themes concerning infectious diseases. 8

10 Improvement of the information provision to the professionals clearly has priority, besides the development of prevention recommendations, such as in the area of vaccinations. Within the Working groups Occupational Allergic Diseases and Industrial Lung and Skin disorders (Northern Region) discussions about individual cases have been an important source of information throughout the years as were the discussions about (new) causes of and the occupational health policy for occupational skin and lung disorders. Further deepening of insight is achieved by lectures about research projects. The way in which healthcare monitoring in working with allergens should take shape was one of the discussion themes this past year. Expertise areas The area of occupational diseases as well as the variety of causes is broad. It is therefore necessary to mark out the areas of expertise in order to offer high quality. Skin disorders, lung disorders, musculoskeletal disorders and emotional disorders within the NCvB are classified as being complementary to assessments provided by the four Knowledge Centers. In a number of other areas however they have a unique character because they fill a gap. The development of three expertise areas has priority at this moment. This concerns infectious diseases and occupation, reproduction (fertility, pregnancy) and occupation and hearing. Cooperation with others is sought where possible and needed. For example, cooperation with the audiology centers of the AMC and the Medical Center of the Vrije Universiteit of Amsterdam (VUmc) is essential for the expertise area of hearing and work. For infectious diseases and occupation it is important that integration takes place between the existing consultation and cooperation structures in the area of infectious diseases, and to add the yet still missing factor occupation. Furthermore for this area of expertise an important role has been put aside for the stimulation of the development of the applicable set of instruments for a preventive policy for occupational healthcare. An information service for matters concerning reproduction and occupation is absent in The Netherlands. To be able to advise the (prospective) pregnant woman well, improvement in transfer of knowledge to care providers, to employers and last but not least to the pregnant woman and her partner is necessary with respect to work-related factors. For the NCvB the dissemination of knowledge is therefore an important activity within this area of expertise. Cooperation with experts in the area of fertility, gynaecology, genetic disorders and reproduction toxicology is needed, both for gathering and for disseminating knowledge. De Werkgroep Infectieziekten en Arbeid (WIZA) kent nog maar een korte historie waarin zij zich bezig heeft gehouden met diverse actuele thema s rond infectieziekten. Verbetering van de informatievoorziening naar de professionals heeft voor haar duidelijke prioriteit, naast het ontwikkelen van aanbevelingen voor preventie, zoals op het gebied van vaccinaties. Binnen de Werkgroepen Allergische Beroepsziekten en Industriële Long- en Huidaandoeningen (Regio Noord- Nederland) zijn door de jaren heen de besprekingen van individuele gevallen een belangrijke bron van informatie geweest en discussie over (nieuwe) oorzaken van en het te voeren bedrijfsgeneeskundig beleid bij beroepshuid- en longaandoeningen. Verdieping vindt verder plaats aan de hand van referaten over onderzoeksprojecten. De wijze waarop de gezondheidsbewaking bij het werken met allergenen gestalte dient te krijgen was het afgelopen jaar één van de discussiethema s. Expertgebieden Het palet van beroepsziekten is breed. Het spectrum van oorzaken is dat evenzeer. Een opsplitsing in expertgebieden is dan ook nodig om als Kenniscentrum voor Beroepsziekten hoogwaardige kwaliteit te kunnen bieden. Voor wat betreft huidaandoeningen, longaandoeningen, aandoeningen van de bewegingsorganen en psychische aandoeningen zijn ze binnen het NCvB aanvullend op expertise die door de vier Kenniscentra voor Arbeidsrelevante Aandoeningen geleverd kan worden. Op een aantal andere gebieden hebben ze echter een uniek karakter omdat zij voorzien in een lacune. Ontwikkeling van een drietal expertgebieden heeft hierbij op dit moment prioriteit. Het betreft infectieziekten en arbeid, reproductie (fertiliteit, zwangerschap) en arbeid en gehoor. Waar mogelijk en noodzakelijk wordt gezocht naar samenwerking met anderen. Zo is het voor gehoor en arbeid essentieel dat er wordt samengewerkt met de audiologische centra van het AMC en VUmc. Voor infectieziekten en arbeid is het van belang dat er integratie plaatsvindt binnen de bestaande overlegen samenwerkingsstructuren op het gebied van infectieziekten, en hier de nu nog ontbrekende factor arbeid in te brengen. Verder is voor dit expertgebied een belangrijke rol weggelegd in het stimuleren van de ontwikkeling van het voor de bedrijfsgezondheidszorg toepasbaar instrumentarium voor een te voeren preventief beleid. Een centraal aanspreekpunt op het gebied van reproductie en arbeid ontbreekt in Nederland. Voor goede advisering aan de (aanstaande) zwangere is verbetering van kennisoverdracht aan zorgverleners, aan werkgevers en niet in de laatste plaats aan de zwangere vrouw en haar partner ten Nederlands Centrum voor Beroepsziekten Jaarverslag 03 Netherlands Center for Occupational Diseases Annual report 03 9

11 aanzien van de relatie met het werk noodzakelijk. Kennisverspreiding is voor het NCvB dan ook een belangrijke activiteit binnen dit expertgebied. Samenwerking met deskundigen op het gebied van fertiliteit, verloskunde, aangeboren afwijkingen en reproductietoxicologie is noodzakelijk, zowel voor de kennisverzameling als voor de kennisverspreiding. Onderzoek Registratie van beroepsziekten Epidemiologisch onderzoek naar beroepsziekten vindt plaats aan de hand van het Peilstation Intensieve Melding (PIM). Dit peilstation bestaat uit een groep van ca. 40 bedrijfsartsen die twee jaar lang met intensieve begeleiding en terugkoppeling beroepsziekten melden aan het NCvB. De verzorgde populaties van de deelnemende bedrijfsartsen aan het peilstation zijn bekend. Desgevraagd leveren de deelnemende bedrijfsartsen extra informatie ten behoeve van het project. De gemelde patiënten worden middels een informed consent gevraagd deel te nemen aan een follow-up onderzoek. Het project is additioneel aan het reguliere registratiesysteem voor beroepsziekten. Aan dit peilstation is een aantal onderzoekstrajecten verbonden. Doelstellingen van het project zijn: 1 Het maken van betere schattingen van de incidentie van een aantal beroepsziekten en hun verdeling in bedrijfstakken, evenals de verdeling van sociaal-demografische parameters; 2 Het inventariseren van de maatschappelijke consequenties van beroepsziekten in termen van gezondheidsschade, economische schade en sociale gevolgen; 3 Het evalueren van de kwaliteit van signalering en diagnostiek van beroepsziekten door bedrijfsartsen; Het project loopt tot en met 2006 en zal naast informatie ten behoeve van nationaal beleid tevens diverse internationale publicaties en een promotie opleveren (onderzoekers: Dick Spreeuwers en Nicole de Wilde). Onderzoek vanuit het Solvent Team Binnen het Solvent Team project worden een aantal onderzoeken uitgevoerd die gericht zijn op verbetering van de diagnostiek en begeleiding van patiënten met chronische toxische encefalopathie (CTE). 1. Evaluatie van het diagnostische proces Op basis van eigen ervaringen en gebruikmakend van buitenlandse expertise worden beslisregels voor de verschillende elementen van het diagnostische proces ontwikkeld (promotieonderzoek Joffrey van der Hoek). Study Registration of occupational diseases Epidemiological studies of occupational diseases are carried out with the Surveillance project for Intensive Notification (PIM) in a central role. This surveillance project consists of a group of approximately 40 occupational physicians, who report occupational diseases to the NCvB for two years with intensive support and feedback. The populations cared for by the occupational physicians participating in the surveillance project are known. On request the participating occupational physicians provide additional information on behalf of the project. The patients who were reported are asked to participate in a follow-up study by way of an informed consent. The project is complementary to the regular registration system for occupational diseases. A number of research projects are connected to this surveillance project. The objectives of the project are: 1 To be able to make better estimates of the incidence of a number of occupational diseases and their distribution in the branches of industry, as well as the distribution of socio-demographic parameters 2 To investigate the social consequences of occupational diseases in terms of health impact, economic and social consequences 3 To evaluate the quality of diagnosis and notification of occupational diseases by occupational physicians. The project runs through 2006 and besides information on behalf of national policy will also result in several international publications and an academic thesis (researchers: Dick Spreeuwers and Nicole de Wilde). Studies initiated by the Solvent Team Within the Solvent Team project a number of studies are carried out that are aimed at the improvement of the diagnostics and support of patients with chronic toxic encephalopathy (CTE). 1. Evaluation of the diagnostic process The rules for decision-making with regard to the different elements of the diagnostic process are developed based on own experiences and by making use of assessment performed/expertise from abroad (academic thesis Joffrey van der Hoek). 2. Evaluation of the effectiveness of the support The effectiveness of the support of patients with CTE, like the one that has been developed within the Solvent Team, is evaluated. In the course of 2004 this support will be turned over to regional rehabilitation centers (promotion research Monique van Hout). 10

12 3. Differential diagnostics CTE and depression A study into the problem of differential diagnostics between depression and CTE has been carried out (academic thesis Ieke Visser). 4. Neuro-imaging and CTE-diagnostics In analogy with a neuro-imaging study in ecstasy users, advanced neuro-imaging studies were carried out in both CTE patients and a control group (academic thesis Ieke Visser in cooperation with the department of Neuroradiology at the AMC). 2. Evaluatie effectiviteit begeleiding De effectiviteit van de begeleiding van patiënten met CTE, zoals die binnen de Solvent Teams is ontwikkeld wordt geëvalueerd. In de loop van 2004 wordt deze begeleiding overgedragen aan regionale revalidatiecentra (promotieonderzoek Monique van Hout). 3. Differentiaal diagnostiek CTE en depressie Er is onderzoek uitgevoerd naar het probleem van de differentiaal diagnostiek tussen depressie en CTE (promotieonderzoek Ieke Visser). 5. Genetic polymorphism The buccal mucosa that was collected from approximately one hundred CTE patients will be analysed. In cooperation with the OPS (Organic Psycho-Syndrome) patient organisation an information session has been organised. Reports will be made about the possible differences in individual sensitivity with regard to the toxicokinetics of solvents (researcher: Dr. M.M. Verberk, toxicologistepidemiologist). Documentation and literature search An important but also difficult assignment for the NCvB is to oversee and, complementary to this, to document the continuous stream of information about occupational diseases. There are numerous sources: scientific magazines, research reports from this country and foreign countries, websites of governmental agencies and research institutes etc. This not only requires a systematic way of monitoring, but also the selection of information about the different areas of expertise. Per subject, the employee responsible for this does a systematic literature search once or twice a year together with the clinical librarian, who besides this also fulfils an important role with monitoring, alerting and documenting. Questions asked to the helpdesk regularly lead to a consultation of the databases, but also manuals are frequently useful. Immediate availability of good and current manuals is an important requirement for functioning well as a knowledge center. The knowledge and information acquired through the questions is not only important to the person asking the questions, but also appears to be relevant enough in certain situations to obtain a broader dissemination via the website (FAQ), the Newsletter, the Dutch Journal for Occupational and Insurance Medicine (TBV) and the Alert Report. An important requirement is that it has an evidence based character. The development of matching search strategies and the critical appraisal of discovered information has priority. 4. Neuroimaging en CTE-diagnostiek In analogie met een neuroimaging studie bij ecstasy gebruikers is bij CTE-patiënten en een controlegroep geavanceerd neuroimaging onderzoek uitgevoerd (promotieonderzoek Ieke Visser in samenwerking met de afdeling Neuroradiologie van het AMC). 5. Genetisch polymorfisme Het wangslijmvlies dat bij een honderdtal CTE-patiënten is verzameld zal worden geanalyseerd. Met medewerking van de OPS-patiëntenvereniging is hiertoe een voorlichtingsmiddag georganiseerd. Gerapporteerd zal worden over de mogelijke verschillen in individuele gevoeligheid i.v.m. de toxicokinetiek van oplosmiddelen (onderzoeker: Dr. M.M. Verberk, toxicoloog-epidemioloog). Documentatie en literatuursearch Een belangrijke maar ook lastige opdracht voor het NCvB is het zicht blijven houden op en in het verlengde hiervan documenteren van de constant aanhoudende informatiestroom over beroepsziekten. De bronnen zijn divers: wetenschappelijke tijdschriften, onderzoeksrapporten uit binnen- en buitenland, websites van overheidsinstellingen en onderzoeksinstituten etc. Het vereist een systematische manier van monitoren maar ook selecteren van informatie over de diverse expertgebieden. Eén tot tweemaal per jaar wordt er per aandachtsgebied door de hiervoor verantwoordelijke medewerker een systematische literatuursearch uitgevoerd samen met de clinical librarian, die daarnaast een belangrijke rol vervult bij het monitoren, signaleren en documenteren. Vragen aan de helpdesk vormen regelmatig aanleiding tot het raadplegen van databases, maar ook handboeken bieden hierbij toch ook nogal eens uitkomst. Directe beschikbaarheid van goede en actuele handboeken is een belangrijke voorwaarde voor het goed kunnen functioneren als kenniscentrum. 13 gen via de website (FAQ), de Nieuwsbrief, het TBV en het signaleringsrapport. Een belangrijke eis is dat zij een evidence based karakter heeft. Het ontwikkelen van hierbij passende zoekstrategieën en kritische waardering van gevonden informatie heeft dan ook prioriteit. De vragen kunnen echter ook een lacune in kennis en informatie zichtbaar maken. Die bevinding kan evenzeer aanleiding zijn tot het afgeven van een signaal. Er mag dan ten aanzien van vele onderwerpen een overdaad aan beschikbare informatie dreigen, een gebrek aan kennis en informatie kan in termen van gezondheidsschade een grotere bedreiging vormen en hiermee aandacht vereisen. Gezien de enorme economische schade die nieuwe infectieziekten zoals SARS en de kippeninfluenza, maar ook de infectieziekten van alle dag met zich meebrengen, is een flinke inspanning op dit gebied zonder meer gerechtvaardigd. In view of the tremendous economic damage caused by infectious diseases like SARS and chicken influenza, but also by every-day infectuous diseases, this situation calls for action.

16 Dissemination of information Informatie verspreiden Websites The website of the NCvB ( offers information about occupational diseases and also offers the possibility of notification of occupational diseases. All publications of the NCvB, including the Alert Report, can be found on the website. In 2003 the NCvB started renewing the website. The objective is to improve the accessibility of information about occupational diseases. Mid 2004 the renewed website will hit the air. The NCvB also made an important contribution to the website This website has been developed together with the Organisation for Applied Scientific Research (TNO) and Work by order of the Association of Insurers (Verbond van Verzekeraars). The site offers an overview of occupations in the building industry and the occupational hazards of these occupations. The site will be expanded with a series of SME occupations in Alert Report on Occupational Diseases 2003 The NCvB yearly draws up the Alert Report on Occupational Diseases by order of the Ministry of Social Affairs and Employment. The report provides an overview on the prevention and dissemination of occupational diseases in our country, as well as the trends in this field. Besides that, it describes new scientific and social developments in prevention, (early) diagnostics, treatment and rehabilitation in cases of occupational diseases. An important objective of this report is giving signals or Alerts about occupational diseases to policy makers as well as to professionals in the field. Alerts are signals about (new) occupational diseases or causes of occupational diseases and about difficulties in the prevention and care of occupational diseases. The report tries to trace as accurately as possible where these difficulties are. In what occupations or sectors do certain occupational diseases occur more frequently than in others and what are the causes for that? Are treatment or prevention possibilities applied adequately or can there be more progress made in preventing occupational damage to health? Websites De website van het NCvB ( biedt informatie over beroepsziekten en biedt tevens de mogelijkheid beroepsziekten te melden. Alle publicaties van het NCvB, inclusief het signaleringsrapport, zijn te vinden op de website. In 2003 is het NCvB gestart met het vernieuwen van de website. Het doel hiervan is het verbeteren van de toegankelijkheid van informatie over beroepsziekten. Medio 2004 zal de vernieuwde website de lucht in gaan. Het NCvB heeft ook een belangrijke bijdrage geleverd aan de website Deze website is samen met TNO-Arbeid ontwikkeld in opdracht van het Verbond van verzekeraars. De site biedt een overzicht van beroepen in de bouw en de arbeidsrisico s in deze beroepen. De site zal in 2004 uitgebreid worden met een reeks MKBberoepen. Signaleringsrapport Beroepsziekten 2003 Het NCvB stelt jaarlijks het Signaleringsrapport Beroepsziekten op in opdracht van het Ministerie van Sociale Zaken en Werkgelegenheid. Het rapport biedt een overzicht over het vóórkomen en de verspreiding van beroepsziekten in ons land, en trends hierin. Daarnaast beschrijft het nieuwe wetenschappelijke en maatschappelijke ontwikkelingen in preventie, (vroeg)- diagnostiek, behandeling en reïntegratie bij beroepsziekten. Een belangrijk doel van dit rapport is het afgeven van signalen of alerts over beroepsziekten aan beleidsmakers en aan professionals in het veld. Alerts zijn signalen over (nieuwe) beroepsziekten of oorzaken van beroepsziekten en over knelpunten in de preventie en zorg voor beroepsziekten. Het rapport tracht zo scherp mogelijk te traceren waar zich deze knelpunten bevinden. In welke beroepen of sectoren komen bepaalde beroepsziekten meer voor dan in andere en wat ligt daaraan ten grondslag? Worden behandel- of preventiemogelijkheden adequaat toegepast of is meer winst te behalen bij het voorkómen van gezondheidsschade door werk? Kennis over beroepsinfectieziekten is nauwelijks aanwezig bij Arbodiensten. Buiten de gezondheidszorg ontbreekt een vaccinatiebeleid gericht op risicovolle functies grotendeels. Beleid bij calamiteiten met beroepsinfectieziekten is onvoldoende.gepleit wordt voor een verbetering van de kennisinfrastructuur van beroepsinfectieziekten en ontwikkeling van beleid ten aanzien van vaccinaties voor risicoberoepen en voor calamiteiten. Arbo-diensten dienen hierbij te worden betrokken. Contamination at work In the past year SARS and fowl plague have made victims in specific professions. Most of the Occupational Health Services have little knowledge about occupational infectious diseases. There is practically no vaccination policy aimed at high-risk positions outside the healthcare sector. The policies in place for calamities involving occupational infectious diseases are inadequate. We are pleading for an improvement in the knowledge infrastructure of occupational infectious diseases and the development of policies regarding the vaccination of high-risk professionals and in the event of calamities. The Occupational Health Services should be involved in this. Ziek van water Nat werk blijkt veruit de belangrijkste oorzakelijke factor voor het krijgen van contacteczeem. Niet alleen oplappen, maar ook zorgen voor goede werkomstandigheden Bij verzuimbegeleiding ligt het accent vaak op individugerichte (activerende) maatregelen. Een activerende en klantgerichte benadering is goed, maar er lijkt te weinig aandacht te zijn voor verbetering van de werkomstandigheden. Dit geldt vooral voor de psychosociale arbeidsomstandigheden, waaronder de arbeidsverhoudingen, van mensen met psychische klachten door het werk. Not merely patching up, but providing healthy working circumstances The focus in absenteeism policies often lies on individually targeted (activating) measures. While an activating and customer-driven approach is good, there seems to be little attention for the improvement of the working conditions. This seems the case particularly for the psycho-social working conditions, including working relations, of people with work-related psychological complaints. PAGO: voorkomen is beter dan genezen Ondanks de verplichting van werkgevers om werknemers een periodiek arbeidsgezondheidkundig onderzoek (PAGO) aan te bieden, worden PAGO s buiten de bouwnijverheid nog maar weinig meer uitgevoerd. Dit terwijl een aantal beroepen een duidelijk verhoogd risico laat zien op bijvoorbeeld gehooraandoeningen, huidziekten of klachten van het bewegingsapparaat. Gepleit wordt voor het ontwikkelen van richtlijnen voor PAGO. Wetenschappelijke evaluatie van dergelijke richtlijnen is van belang of zij daadwerkelijk bijdragen aan preventie van gezondheidsschade door werk. Periodic Examination (PAGO): it s better to prevent than to cure Despite employers obligation to offer employees a Periodic Examination (PAGO), few PAGOs are carried out. Only in the construction industry are they carried out frequently, despite the fact that a number of professions are showing a clear increase of the risk of, for example, hearing loss, skin diseases or musculoskeletal diseases. Guidelines for periodical examinations should be developed. A scientific evaluation of these guidelines is important to establish if they actually contribute to the prevention of work-related health disorders. 16

18 Helpdesk of the Netherlands Center for Occupational Diseases In 2003 the NCvB received 723 questions. Both by telephone and by . The latter occurred increasingly and is logistically easier than telephone contact between busy people. Occupational physicians are the largest category of people asking questions; besides that, questions are asked by amongst others policy employees of ministries, branch organisations etc., and also other medical and Occupational Health Service consultants. The number of questions to the NCvB is rising (in 2002 there were 544 questions). The table below provides an overview of people asking questions. Helpdesk Nederlands Centrum voor Beroepsziekten In 2003 kwamen bij het NCvB 723 vragen binnen. Zowel telefonisch als via de elektronische weg. Dit laatste gebeurt in toenemende mate en is logistiek eenvoudiger dan het telefonisch contact tussen druk bezette mensen. Bedrijfsartsen vormen de grootste categorie vragenstellers; daarnaast worden vragen gesteld door o.a. beleidsmedewerkers van ministeries, brancheorganisaties e.d., andere medici en arbo-adviseurs. Het aantal vragen dat aan het NCvB wordt gesteld is stijgend (in vragen). In tabel 3 een overzicht van de vragenstellers. Important because it is a method through which knowledge about occupational diseases is disseminated in the most efficient way. Important also because the problem of the person asking questions could be a signal of a specific case or of a more universal problem, which in turn could provide input for further study, articles in the Newsletter, the Alert report, FAQ s on the website or case reports for educational purposes. A reaction to the question is given within 48 hours. The questions vary from simple questions of knowledge to complex problems for which, in order to answer them, a literature search or consultation with an external expert is needed. Some of the questions are transferred to one of the knowledge centers of the Knowledge Network for Health and Occupation immediately. Below you will find an overview with the numbers divided into categories with an anthology of the questions. Het goed beantwoorden van vragen over beroepsziekten is een belangrijke NCvB-activiteit. Newsletters, TBV Each quarter the NCvB prints a Newsletter. Via the Newsletter ARBO professionals are kept up to date of the developments in the field of occupational diseases. Traditionally the back page of the Newsletter is intended for the case reports on occupational diseases. These case reports have an educative function. Special cases or new occupational diseases can also be presented. Each month the NCvB places a page in the Magazine for Occupational and Insurance Healthcare (TBV). Here too an educative and informative function is pursued. Heijermans lectures Twice per year the NCvB organises a Heijermans lecture. Approx. 150 occupational physicians and others, who are interested, attend the Heijermans lecture. The first Heijermans lecture with the theme Occupational diseases across the borders took place on 20 June. Contributions were made by France and Germany and the visitors of the lecture received an impression of how other countries in Europe dealt with occupational diseases. The second Heijermans lecture took place on 12 December and discussed the subject Occupational back complaints. During this lecture the concept criteria document for workrelated non-specific low back complaints was presented, which was drawn up in a cooperative project of the Netherlands Knowledge Center for Occupation and Complaints of the Musculoskeletal System (AKB), the Coronel Institute and the NCvB. Het NCvB verzorgt iedere maand een pagina in het Tijdschrift voor Bedrijfs- en Verzekeringsgeneeskunde (TBV). Ook hier wordt een educatieve en informatieve functie nagestreefd. Heijermanslezingen Tweemaal per jaar organiseert het NCvB een Heijermanslezing. De Heijermanslezing wordt zeer goed bezocht door ca. 150 bedrijfsartsen en andere belangstellenden. De eerste Heijermanslezing vond plaats op 20 juni met het thema Beroepsziekten over de grenzen. Bijdragen werden geleverd uit Frankrijk en Duitsland en de bezoekers van de lezing kregen een beeld hoe in andere Europese landen omgegaan wordt met beroepsziekten. De tweede Heijermanslezing vond plaats op 12 december en had als onderwerp Arbeidsgebonden rugklachten. Tijdens deze lezing werd het concept criteriadocument voor arbeidsgerelateerdheid van aspecifieke lage rugklachten gepresenteerd, dat in een samenwerkingsproject van het Nederlands Kenniscentrum Arbeid en Klachten Bewegingsapparaat (AKB), het Coronel Instituut en het NCvB is opgesteld.
