1 GGZ Internationaal De afgelopen 2,5 jaar heeft GGZ Nederland de Europese ggz verkend. Welke internationale spelers en netwerken zijn relevant voor het werk van leden en GGZ Nederland? Welke activiteiten hebben meerwaarde? Eerder dan verwacht leidde dat tot resultaten bij de WHO en OECD. Netwerken zijn in kaart gebracht en opgebouwd. Ook is een start gemaakt met overdracht van kennis, ervaringen en netwerken naar de leden van GGZ Nederland. Eind 2013 hebben de lidorganisaties aangegeven dat GGZ Nederland internationaal een actieve rol moet blijven vervullen, gericht op beïnvloeding van (inter)nationaal ggz-beleid en kennisuitwisseling. Bouwend op de ervaringen uit de afgelopen 3 jaar, kiest het bestuur voor deze hoofdlijnen in de periode : Bouwen aan een krachtige Europese koepel voor geestelijke gezondheid en het verder ontsluiten relevante internationale netwerken voor leden. Consolideren van de opgebouwde relaties en reputatie bij internationale organisaties. Versterken van de uitwisseling van kennis met de leden. Uitbesteden van werkzaamheden die beter en goedkoper door derden kunnen worden uitgevoerd. Het gaat dan vooral om informatie over Europees beleid en subsidies. Een verslag van de afgelopen 2,5 jaar staat in bijlage 1. De uitkomsten van een onderzoek onder leden naar de meerwaarde staan in bijlage 2. In bijlage 3 een verslag van de activiteiten van de European Alliance for Mental Health in all Policies. Belangenbehartiging, lobby en reputatiemanagement Bedoeling Leden vinden het belangrijk dat GGZ Nederland internationale ontwikkelingen actief beïnvloedt en niet alleen passief volgt. Zij zien meerwaarde in beleidsbeïnvloeding van Europese instellingen door GGZ Nederland. Het EU beleid en ervaringen met beleid in andere landen zijn ook bruikbaar om de gedachtenvorming en positie van Nederlandse stakeholders te beïnvloeden. GGZ Nederland zet deze U-bocht in om het Nederlands beleid in de door leden gewenste richting te beïnvloeden. Afstemming tussen Haagse en Brusselse lobby blijft daarbij een belangrijk uitgangspunt. Hoofdlijnen Met WHO Europa en de OESO is een goede relatie opgebouwd. Het bestendigen van deze relatie de komende jaren is nodig, wil GGZ Nederland in in een vroeg stadium betrokken zijn bij de beleidscyclus en planvorming na Zo houdt GGZ Nederland goed zicht op internationale ontwikkelingen die mogelijk gevolgen kunnen hebben voor leden. 1

2 Het netwerk binnen de EU is nu nog beperkt, de uitbouw daarvan heeft de komende periode prioriteit. Om als Nederlandse organisatie impact te hebben, is structurele samenwerking nodig met relevante Europese partners. Naast de belangrijkste partner in Brussel (Mental Health Europe) blijft het noodzakelijk om samenwerking te zoeken met andere netwerken en samenwerkingsverbanden van bijvoorbeeld verzekeraars, werkgevers en zorgaanbieders. Plan voor 2015 Mental Health Europe is op dit moment het enige Europese netwerk voor geestelijke gezondheid die patients, professionals, providers and policy makers verenigen. Inhoudelijk richt MHE zich vooral op mensenrechten, herstelgericht werken, deinstitutionalisering, werk en stigmabestrijding. In 2014 zijn het beleid en de organisatie van MHE tegen het licht gehouden, de uitkomsten daarvan komen in het voorjaar in de ledenvergadering. GGZ Nederland trekt de betreffende werkgroep en werkt gericht aan het positioneren van MHE als het netwerk voor geestelijke gezondheid in Europa. Stappen: GGZ Nederland blijft actief betrokken bij het beleid en organisatie van MHE. Na het definitief vaststellen van de strategie en de organisatie van MHE (voorjaar 2015) kijkt het bestuur van GGZ Nederland naar de invulling van haar rol in het bestuur van MHE. GGZ Nederland heeft in overleg met MHE het initiatief genomen voor een European Alliance for Mental Health in all Policies. In 2013 kon MHE dit initiatief niet zelf trekken vanwege lopende Europese projecten. MHE nam de activiteiten van de Alliance wel op in haar werkplan voor 2014, maar de Europese Commissie kende voor deze activiteiten geen subsidie toe. GGZ Nederland zet het initiatief komend jaar wel voort. Dit zijn de overwegingen: Op dit moment nemen AEIP (pensioen en sociale verzekeraars), AIM (zorgverzekeraars), EPHA (publieke gezondheid), Mental Health Europe (MHE) naast GGZ Nederland deel aan de alliantie. Inmiddels is op Europees niveau wel veel meer aandacht voor het belang van geestelijke gezondheid en de maatschappelijke baten en economische waarde van de ggz. Dit leidt nog niet bij alle EU departementen tot aanpassing van beleid. Er is nog veel werk te verzetten. De weerklank bij de verschillende Europese koepels en bij het Directoraat Gezondheid is wel groot. De alliantie sluit heel goed aan bij lopende discussies in Europa. De alliantie is voor GGZ Nederland een mooi vehikel om kennis te maken met Europese partijen. In het Europese krachtenveld is GGZ Nederland niet de voor de hand liggende speler om gedurende meerdere jaren een Europese Alliantie te trekken. MHE heeft de wens uitgesproken om deze leidende rol op termijn te vervullen. Stappen: Consolideren alliantie en vergroten draagvlak bij deelnemers: sterk inhoudelijk programma. Aansluiten bij en beïnvloeden van lopende EU projecten (Joint Action on Mental Health) Daarna gericht uitbreiden met partners buiten de zorg, zoals vakbonden en werkgevers. Overdragen rol van trekker aan Europese organisatie. 2

3 Kennisoverdracht aan leden van GGZ Nederland over Europees beleid en subsidies Bedoeling Leden hebben de wens om op de hoogte te blijven van Europees beleid met mogelijke impact op de bedrijfsvoering. Leden willen ook graag informatie krijgen over Europese subsidies. Hoofdlijnen De afgelopen 2,5 jaar bleek een uitputtende overdracht van kennis over Europees beleid en subsidies niet mogelijk. De Europese Unie heeft meer dan 100 subsidieregelingen. GGZ Nederland zoekt met andere branches naar een passende invulling van deze wens van leden. GGZ Nederland mikt binnen 3 jaar op een oplossing waarbij onze leden (en die van samenwerkende branches) één goed toegankelijk, betrouwbare en betaalbare bron van informatie hebben. Plan voor 2015 Voorjaar 2014 is gesproken met het Vlaamse Kenniscentrum Sociale Economie (KCSE). Dit centrum detecteert, vertaalt en analyseert Europees beleid en informeert en adviseert Vlaamse instellingen en overheden over de impact op Vlaamse wet- en regelgeving. GGZ Nederland kan zich aansluiten om deze informatie beschikbaar te maken voor leden. Leden van GGZ Nederland kunnen desgewenst een breder pakket diensten afnemen, zoals toegang tot standaard informatie over subsidies, leggen van contacten met deelnemers aan Europese consortia en het volledig begeleiden van de aanvraag en administratieve afwikkeling van subsidies. De vertaling naar Nederlandse wet- en regelgeving ontbreekt dan nog. Dat is alleen uitvoerbaar als meerdere branches op dit onderwerp samenwerken. De kennis en ervaring van KCSE kan een goede basis zijn, KCSE ziet zelf ook voordelen in de samenwerking met een Nederlandse variant. Stappen: GGZ Nederland overleg met KSCE over een model van dienstverlening. Als dit gesprek goed verloopt, sluit GGZ Nederland zich voor een periode van 3 jaar aan bij KCSE. Aan het eind van het 2 e jaar volgt een evaluatie met de leden van GGZ Nederland op de waarde van deze diensten. Tegelijkertijd gaat GGZ Nederland met andere Nederlandse branches verkennen of het zinvol en wenselijk is om gezamenlijk een Nederlandse variant van het KCSE op te zetten. Dit in nauwe samenwerking met KSCE. Internationale netwerken voor uitwisseling van kennis Bedoeling De uitwisseling met organisaties buiten Nederland is een belangrijke bron van inspiratie en kennis voor leden van GGZ Nederland. In de uitwisseling kan de Nederlandse geestelijke gezondheidszorg ook mooie voorbeelden en resultaten delen met partners in het buitenland. Hoofdlijnen GGZ Nederland verkent in Europa welke netwerken van toegevoegde waarde kunnen zijn voor leden, informeert de leden hierover en legt desgewenst contact met voor hun relevante organisaties. 4 transfer van kennis uit internationale netwerken naar de leden is een belangrijk aandachtspunt voor de komende periode. Deze overdracht van kennis naar leden vindt nu plaats via GGZ Connect en ledennieuwsbrieven. Plan voor 2015 Voor de uitwisseling van kennis en ervaringen in de ggz is het International Initiative of Mental Health Leaders (IIMHL) een inhoudelijk sterk netwerk dat door cultuur en taal goed toegankelijk is voor leden. Nederlands is op dit moment geen lid. Bestuurders kunnen gratis lid worden, zij kunnen medewerkers autoriseren om kennis te nemen van de ontwikkelde producten en deel te nemen aan gezamenlijke uitwisselingsprogramma s. De kosten zijn laag, de impact voor leden relatief hoog. Stappen: GGZ Nederland gaat in gesprek met VWS over het lidmaatschap van IIMHL. GGZ Nederland brengt het IIMHL onder de aandacht van bestuurders van leden in opmaat naar de internationale uitwisseling in september EuroHealthNet is een stevige stakeholder in Brussel in public health. De inzet op bevordering en preventie (geestelijke gezondheid) past bij de meerjarenvisie GGZ Nederland. Tot 2013 was het lidmaatschap van EuroHealthNet beperkt tot overheidsinstanties, sinds 2014 is een deel van het netwerk open voor derden. In het onderdeel PHASE staat belangenbehartiging centraal, in CIRI kennisnetwerken en onderzoek. De kosten voor lidmaatschap bedragen voor elk netwerk 500 per jaar. Geestelijke gezondheidszorg is (nog) niet expliciet in beeld bij EuroHealthNet, dat heeft wel belangstelling voor dit onderwerp. Lidmaatschap biedt GGZ Nederland de gelegenheid om met een relatief kleine investering geestelijke gezondheid op de agenda van deze organisatie te krijgen en te houden. Het lidmaatschap biedt ook toegang tot relevante netwerken bij (semi)overheden. Stappen: GGZ Nederland gaat met EuroHealthNet in gesprek over aansluiting bij een van de netwerken. GGZ Nederland sluit zich voor een periode van 2 jaar aan bij EuroHealthNet. GGZ Nederland evalueert de waarde van dit netwerk voor afloop van deze periode. GGZ Nederland blijft daarnaast in gesprek met internationale partners en verkent relevante en interessante ontwikkelingen voor lidinstellingen. Zodra de mogelijkheid zich voordoet, verbindt GGZ Nederland haar leden met partners in het buitenland. VNO NCW organiseert 1 à 2 maal per jaar een Europees overleg zorg met sprekers uit diverse Europese instanties. Daarnaast stemt GGZ Nederland af over de impact Europees beleid (privacy; structuurfondsen). VNO NCW heeft een groot netwerk en kan voor specifieke onderwerpen contact leggen met Europese partners. Zij vervult dan de rol van wegwijzer. De transfer van kennis naar lidinstellingen vraagt nadere aandacht. Dit is ook een vraag die past binnen het onderdeel kennisbeleid. In afstemming met de afdeling communicatie zal worden gekeken welke vorm / welk platform deze kennistransfer het beste faciliteert. 4

5 Bijlage 1: Resultaten Wat was het plan in 2012 Het bestuur van GGZ Nederland had een duidelijke ambitie: in 2015 betrokken zijn bij de plan- en besluitvorming in Europa over geestelijke gezondheidszorg. De leden wilden betere toegang tot relevante netwerken om kennis te brengen en te halen. Waarom deze ambitie? De EU bestempelt medische zorg tot dienst en krijgt daarmee steeds meer invloed op het beleid van de lidstaten. Een voorbeeld is het Europees semester, waar regeringen intensief met elkaar overleggen over economisch beleid. Iedere lidstaat moet de aanbevelingen van de commissie verwerken in beleid en begroting. Zo vindt de Europese Commissie dat Nederland de zorg te veel heeft geformaliseerd. Dit leidde tot de ingrijpende transities in de langdurende zorg. De EU ontwikkelt beleid dat ondernemers en werkgevers in de zorg direct raakt: mensenrechten; privacy; erkenning van opleidingen en diploma s; patiëntveiligheid; arbobeleid; voedselveiligheid en (uitzonderingen op) mededingingsrecht. De Nederlandse ggz staat voor ingrijpende veranderingen. Sommige zijn elders in Europa al verder (ambulantisering), op andere gebieden loopt de Nederlandse ggz voor (emental Health; transparantie). Om gericht kennis te halen en te brengen dient de branche deel te nemen aan relevante netwerken. Europa biedt Nederlandse zorgondernemers kansen. GGZ Nederland verbindt leden desgevraagd met relevante netwerken in Europa. Het bestuur koos bewust voor een beperkte en beheerste aanpak: GGZ Nederland wil Europa inzetten voor imagomanagement en belangenbehartiging (U-bocht). Centraal thema is positieve politieke aandacht voor geestelijke gezondheid en voldoende financiële ruimte voor ggz in Europa en Nederland. Zij zoekt de juiste partners en bouwt daar bestendige relaties mee op. GGZ Nederland zoekt de verbanden tussen Europees beleid en (strategische) dossiers, bepaalt de impact op leden en communiceert de uitkomsten naar de leden. Zo kunnen GGZ Nederland en de leden anticiperen op de impact van Europees beleid op de bedrijfsvoering. Internationale benchmarks bepalen mede het beeld van de Nederlandse ggz en beïnvloeden zo ambtelijke en politieke meningsvorming in Den Haag. GGZ Nederland werkt (inter)nationaal aan betrouwbare en betekenisvolle benchmarks. GGZ Nederland maakt de Nederlandse ggz zichtbaar in het buitenland. GGZ Nederland bundelt kennis, ervaring en netwerken en communiceert hierover regelmatig met leden. Zij faciliteert zo zorgondernemers in de ggz. GGZ Nederland en haar leden dragen bij aan de ontwikkeling van de ggz in andere landen door uitwisseling van kennis. GGZ Nederland is enkele jaren nauwelijks aanwezig geweest in Europa, daarom start GGZ Nederland met de volgende randvoorwaarden: verzamelen, opstellen en publiceren Engelstalige informatie over Nederlandse ggz; inventariseren behoeften en wensen van leden en hun internationale contacten; promoten beschikbare netwerken en bijeenkomsten bij leden. 5

6 Realisatie De actualiteit noopte al snel tot het bijstellen van dit plan. Internationale organisaties startten al in 2012 met de beleidvorming voor , GGZ Nederland legde hier de nadruk eerst op. De afgelopen 2 jaar vroeg ook Mental Health Europe meer tijd dan verwacht. Hierdoor bleef de informatievoorziening richting leden achter. Hieronder de uitkomsten van de afgelopen 2,5 jaar. Belangenbehartiging, lobby en reputatiemanagement succesvol World Health Organization Rapporten van de WHO vormen vaak een basis of ondersteuning voor Europees of Nederlands beleid. WHO Europe stelde in september 2013 het Europese Mental Health Action Plan vast. Sinds april 2012 heeft GGZ Nederland (samen met VWS) op de concepten gereageerd. De WHO heeft de voorstellen vrijwel allemaal verwerkt. De definitieve tekst sluit aan bij het bestuurlijk akkoord uit juni 2012 en de recente meerjarenvisie van GGZ Nederland. Niet alleen beweegt de Nederlandse ggz zich inhoudelijk in de goede richting, vanuit de WHO zijn de komende jaren geen grote inhoudelijke verrassingen te verwachten. OESO/OECD In maart 2012 kregen we een concept -benchmark over de efficiëntie van ggz-systemen van de Organisatie voor Economische Ontwikkeling en Samenwerking (OESO) onder ogen: In contrast Netherlands, Ireland and Czech Republic tend to emerge relatively consistently as the poorer performers. De figuur illustreert de positie van Nederland op plaats 30 van de 30 voor efficiëntie. GGZ Nederland is in overleg met OECD en VWS gegaan over de gebrekkige onderbouwing van deze benchmark en vervolgens hoe te komen tot een goed onderbouwde analyse van de Nederlandse ggz. De OECD was het eens met onze kritiek en nodigde GGZ Nederland uit voor het leveren van een bijdrage. Medewerkers van GGZ Nederland schreven de teksten voor de belangrijkste thema s. Het uiteindelijke rapport is juli 2014 gepubliceerd en heel goed ontvangen. Ook het overkoepelende rapport Making Mental Health Count ondersteunt de kernboodschappen van GGZ Nederland: geestelijke gezondheidszorg is een investering in de samenleving, overheden investeren te weinig. GGZ Nederland draagt sinds september 2012 ook bij aan het rapport van de OESO over Mental Health at Work via gesprekken en reviews van concepten. Het rapport is in november aan de Nederlandse overheid aangeboden en heeft de nodige publiciteit gekregen. 6

7 IIMHL GGZ Nederland is sinds 2009 betrokken bij het indicatoren project van de IIMHL. Twaalf landen werken aan consensus over indicatoren om prestaties op het gebied van ggz te kunnen meten. De 2 e fase van het project (selectie indicatoren) is voorjaar 2013 afgerond, de geselecteerde indicatoren passen grotendeels bij de gegevens die leden nu al verzamelen. Fase 3 van dit project loopt op dit moment (testen haalbaarheid, betrouwbaarheid en zeggingskracht; ontwikkelen indicator herstel). In juni 2014 woonde GGZ Nederland de IIMHL conferentie bij. Deze 16-maandse uitwisseling kan van grote waarde zijn voor Nederlandse organisaties en hun leiders als het gaat om kennisuitwisseling. Europese Unie GGZ Nederland heeft contact gelegd met de belangrijkste departementen van de Europese Unie (Gezondheid, Sociale Zaken en Werkgelegenheid, Communicatie). Daarnaast heeft GGZ Nederland samen met partners (met wisselend succes) bijgedragen aan Europese consultaties. GGZ Nederland en enkele leden deden mee aan de raadpleging over een nieuwe strategie voor Gezondheid en Veiligheid op het Werk ( ) van de Europese Commissie. Aanleiding was de evaluatie van het beleid tussen , met geestelijke gezondheid in de zijlijn. Samen met LPGGZ, AEIP, MHE en EPHA heeft GGZ Nederland tekortkomingen in beeld gebracht en een antwoordschema opgesteld. Partijen namen elkaars punten over. De Europese Commissie leg in haar nieuwe beleid op dit gebied veel meer nadruk op geestelijke gezondheid, de thema s die de alliantiepartners hebben aangedragen zijn duidelijk zichtbaar in het nieuwe beleid. GGZ Nederland heeft ook samen met Nederlandse partners een bijdrage geleverd aan de Europese consultaties voor patiëntveiligheid en het groenboek voor mhealth. De uitkomst van de eerste consultatie beperkt zich slechts tot de uitspraak: er moet meer aandacht komen voor geestelijke gezondheid. De uitkomsten van de consultatie over mhealth zijn nog niet bekend. GGZ Nederland neemt deel aan de Joint Action on Mental Health and Well-being van de Europese Unie, over geestelijke gezondheid en werk. Veel leden hebben bijgedragen aan de sterkte-zwakte analyse voor Nederland. Daarnaast zet GGZ Nederland haar expertise als werkgever op dit gebied in. GGZ Nederland draagt bij aan de Europese conferentie van deze Joint Action in oktober Versterken Europese koepel MHE Geestelijke gezondheid heeft geen prioriteit in Europa. GGZ Nederland is als nationale organisatie geen natuurlijke partner voor Europese instituties. GGZ Nederland investeerde de afgelopen tijd daaarom vooral van de Europese koepel Mental Health Europe (MHE). Mental Health Europe is de enige onafhankelijke organisatie in Brussel die zich exclusief richt op geestelijke gezondheid. Sinds het voorjaar van 2012 maakt GGZ Nederland weer deel uit van het bestuur van MHE, een vertegenwoordiger van GGZ Nederland is penningmeester en lid van het dagelijks bestuur. Na afronding van de langlopende Europese programma s eind 2013, heeft het bestuur pas op de plaats gemaakt en het beleid en de organisatie van MHE tegen het licht gehouden. De uitkomsten daarvan komen in het voorjaar van 2015 in de ledenvergadering van MHE. GGZ Nederland trekt de betreffende werkgroep en werkt gericht aan het positioneren van MHE als het netwerk voor 7

8 geestelijke gezondheid in Europa. Het bureau van GGZ Nederland heeft met andere grote leden van MHE gesproken over de koers voor de komende jaren. In april 2014 stelde de ALV de strategie vast met als speerpunten mensenrechten, herstelbenadering, deinstitutionalisering, werk en stigmabestrijding. Bouwen aan een European Alliance for Mental Health in all Policies Vanaf het voorjaar van 2013 werkte GGZ Nederland samen met MHE aan een Europese alliantie om geestelijke gezondheid(szorg) hoger op de beleidsagenda s van de EU en haar lidstaten te plaatsen. Het doel is dat krachtige Europese beïnvloeders organisaties van werkgevers en werknemers, pensioenfondsen en uitkeringsinstanties deze boodschap overnemen: bezuinigen op ggz raakt niet alleen patiënten, maar kost de economie veel meer geld door lagere arbeidsproductiviteit, hogere uitkeringen en hogere kosten voor somatische gezondheidszorg. Deze lobby sluit goed aan bij recente publicaties. In bijlage 3 staat het inhoudelijke verslag van de afgelopen anderhalf jaar. Aanvullend op dit verslag: De gesprekken met European Public Health Alliance (EPHA), Association Européenne des Institutions Paritaires (koepel paritaire organisaties, zoals pensioenfondsen) en MHE over een gezamenlijke verklaring zijn positief afgerond. In september 2014 sloot ook de Europese koepel van zorgverzekeraars (AIM) aan. De gesprekken over de Alliantie hebben binnen de werkgroep preventie van AIM al geleid tot een keuze voor geestelijke gezondheidszorg als een van de speerpunten van de komende jaren. De volgende stap is uitbreiding van de alliantie met partijen buiten de gezondheidszorg. De Europese Commissie volgt dit initiatief met veel belangstelling, het departement Gezondheid heeft GGZ Nederland en MHE uitgenodigd voor een gesprek met een delegatie van het departement. Ook is een bijeenkomst in planning met de Europese interdepartementale ggzgroep, waar 6 directoraten zitting in hebben. Opbouwen netwerken voor kennisuitwisseling in volle gang GGZ Nederland heeft de afgelopen jaren in Vlaanderen, de Angelsaksische landen, Oostenrijk en Duitsland een stevig netwerk opgebouwd. In andere Europese landen is een eerste verkenning geweest (Italië, Scandinavische landen, Frankrijk, Baltische staten). GGZ Nederland verbindt daarnaast ook leden en maatschappelijke partners (Fonds Psychische Gezondheid, Samen Sterk zonder Stigma, LPGGZ) met netwerken in Europa. Op het gebied van transparantie en emental Health loopt Nederland internationaal voorop. De Nederlandse organisatie van de ggz is onderwerp van gesprek tijdens werkbezoeken en congressen, ook publiceert het bureau van GGZ Nederland regelmatig artikelen over deze onderwerpen. Een overzicht van internationale publicaties en voordrachten: In een artikel in Science Omega Review van juli 2013 plaatsten Chris Nas en Alex van Geldrop de het Nederlandse systeem in de context van actuele internationale ontwikkelingen (WHO, EU). In de WHO-publicatie "Successes and Failures of Health Policy in Europe" uit 2013 beschrijven de auteurs Routine Outcome Monitoring in Nederland als voorbeeld. 8

9 In september 2013 presenteerde GGZ Nederland in 2 bijeenkomsten van AEIP de Europese alliantie aan Europese pensioenfondsen en sociale verzekeraars. MHE gaf 26 september 2013 een presentatie in het Europees Parlement over emental Health. MHE verwees op advies van GGZ Nederland naar diverse Nederlandse voorbeelden. Tijdens het 8 e European Congress on Violence in Clinical Psychiatry van oktober 2013 presenteerde GGZ Nederland producten van het patiëntveiligheidsprogramma, zoals de kaartenset voor de analyse en preventie van agressie en de handreiking fysiek veilig ingrijpen. In januari 2014 leverde GGZ Nederland samen met LPGGZ een bijdrage aan een WHO conferentie in Lille over de participatie van cliënten in de zorg in projecten ROM en patiëntveiligheid. In juli 2014 verschenen de OECD rapporten Making Mental Health Count en Mental Health analysis Profile: The Netherlands. Deze rapporten geven een evenwichtig beeld van de Nederlandse ggz. Tijdens het 3 e Europees Congres voor Geestelijke Gezondheid in Tallinn van september 2014 gaf het bureau van GGZ Nederland 2 voordrachten, over het Nederlandse ggz-stelsel en ROM. Tijdens de jaarvergadering van het Dachverband Gemeinde Psychiatrie was GGZ Nederland key note speaker over maatschappelijke baten van de ggz en de veranderingen in de ambulante zorg. Eind september vroeg de clinical director mental health van NHS England aan GGZ Nederland naar de ervaringen met en gegevens over het beleid ten aanzien van wachtlijsten. Op 8 oktober maakte minister Norman Lamb de normen voor wachttijden in de ggz bekend. Tijdens een buitengewone ledenvergadering van AIM op 14 november 2014 gaf GGZ Nederland voor 50 Europese ziekenfondsen/zorgverzekeraars een overzicht van actuele ontwikkelingen in de ggz in Europa en een toelichting op de European Alliance for Mental Health in All Policies. Op uitnodiging gaf GGZ Nederland tijdens het jaarlijkse congres van DGPPN in Berlijn een presentatie over het Nederlandse systeem van prestatie-indicatoren en uitkomsten tot nu toe van het IIMHL project. Buitenlandse experts komen ook naar Nederland. Tijdens de GGZ Kennisdag 2013 verzorgde de programmaleider ggz van WHO Europa een presentatie over ambulantisering. Ook waren er voorbeelden van de ontwikkelingen in Italië, Engeland en Oostenrijk. In november 2013 organiseerde GGZ Nederland voor bestuurders een bijeenkomst over internationale samenwerking, met als spreker Graham Thornicroft. In februari 2014 gaven twee OESO onderzoekers op de GGZ Kennisdag een toelichting op hun onderzoek naar geestelijke gezondheid op het werk. Het IIMHL maakte het mogelijk dat Ken Jue uit de VS een presentatie kon geven over zijn programma InShape voor circa 50 mensen. Uitnodigingen voor buitenlandse congressen worden gedeeld met leden. Leden van GGZ Nederland namen deel aan de bijeenkomsten van Mental Health Europe in 2012 en 2013 en de bijeenkomst van de Permanente Vertegenwoording van Nederland over ehealth in april Voor leden die dat aangeven, brengt het bureau van GGZ Nederland contacten tot stand gebracht met relevante Europese organisaties. Daarnaast heeft het bureau bijgedragen aan diverse werkbezoeken aan Nederland (Mental Health Europe; Zorgnet Vlaanderen; Canadese politie). 9

10 Europese subsidies Het bureau heeft een eerste inventarisatie gemaakt van de mogelijkheden om Europese subsidies te verwerven. Nu heeft de Europese Unie meer dan 100 subsidieprogramma s. Geestelijke gezondheid is de komende 7 jaar geen prioriteit, de nadruk ligt op actief en gezond ouder worden en de thema s alcohol, tabak en obesitas. De kans op toekenning van subsidie is verder niet groot (circa 10%). Verder is in 2013 onderzocht of de Europese Regionale Fondsen voor leden interessant kunnen zijn. Hoewel gezondheid een speerpunt is, past specialistische ggz niet goed binnen de criteria. Het Nederlandse kabinet legt in overleg met Europese Commissie de nadruk op innovatie in het MKB en milieu. In andere Europese landen komen projecten in de ggz wel in aanmerking (ambulantisering). De verkenning leerde vooral dat het voor GGZ Nederland niet haalbaar is om al deze programma s te screenen op relevantie voor leden. Toch biedt Europa leden mogelijkheden om te innoveren. Daarom is wel onderzocht naar alternatieven om leden te informeren over de mogelijkheden. Het bureau sprak daartoe voorjaar 2014 met het Vlaamse Kenniscentrum Sociaal Europa. Dit initiatief van Vlaamse organisaties voor zorg en welzijn signaleert nieuw Europees beleid en calls voor Europese subsidies. Zij maken analyses en stellen deze via een beveiligde website beschikbaar. GGZ Nederland kan daar naar verwachting ook gebruik van maken. Elk lid van GGZ Nederland zich voor een gering bedrag abonneren op dienstverlening op het gebied van subsidies. In het najaar van 2014 krijgt deze verkenning een vervolg. Randvoorwaarden zijn vervuld De inventarisatie onder leden van augustus 2013 laat een breed draagvlak zien voor het beleid dat het bestuur heeft uitgezet. Circa 90% is van mening dat GGZ Nederland leden moet informeren over ontwikkelingen in Europa en het beleid op het gebied van ggz actief moet beïnvloeden (bijlage 2). Via GGZ Connect, ledennieuwsbrieven en lobby mail hebben leden toegang tot informatie over ontwikkelingen, bijeenkomsten en congressen. De Engelstalige webpagina en vertaalde documenten geven een compleet overzicht van de Nederlandse ggz. Deze pagina wordt steeds beter gevonden, tussen 1 januari en 14 september 2014 vonden 384 mensen deze pagina gericht via de homepage. In deze periode bezochten circa mensen uit 99 landen de website van GGZ Nederland. Binnen het bureau van GGZ Nederland is het internationaal perspectief breder verankerd. Dit geldt in het bijzonder voor dossiers als ambulantisering, arbeidsomstandigheden, dwang en drang, emental Health, informatiebeleid, langdurende zorg, opleidingen, patiëntveiligheid en privacy. Tot slot maakte GGZ Nederland in 2013 afspraken met het Global Initiative on Psychiatry (GIP) over internationale (ontwikkelings)samenwerking. Deze afspraken kwamen tot stand na een evaluatie in het voorjaar van Daaruit bleek dat de inspanningen van Nederlandse ggz-organisaties in de Baltische staten hadden geleid tot een blijvende verbetering van de ggz in deze landen. De samenwerking had ook toegevoegde waarde voor de organisaties die een bijdrage leverden. 10

13 Bijlage 3: European Alliance on Mental Health in all Policies Introduction In close cooperation with several European partners, GGZ Nederland started an initiative to setting of mental health under the working population. On the basis of a working document, GGZ Nederland explored interests and possible activities and rallied to gather like-minded. These informal consultations continued until late 2013, while the initial discussion document was adapted. Now, 1.5 years later, the initiative has led to an alliance of like-minded and created a dynamic of joint interests and joint actions. This activity report witnesses developments and activities of the alliance. Context Estimates indicate that the total costs of work-related mental health diseases and conditions in the EU27 are nearly 610 billion per annum. The major impact is suffered by: the employers due to absenteeism and presenteeism ( 270 billion) the economy in terms of lost output ( 240 billion) the healthcare systems due to treatment costs ( 60 billion) the social welfare systems due to disability benefit payments ( 40 billion) persons with mental health problems who suffer from a serious illness and are at risk for poverty. The economic consequences of mental health problems mainly in the form of lost productivity are estimated to average 3 4% of gross national product in European Union (EU) countries. Because severe mental disorders often start in adolescence or young adulthood, the loss of productivity can be long lasting. Mental disorders account for more than one third of the years lived with disability in the WHO European Region. As such, mental health is an asset for our economy, not a burden on Member States budgets. Although evidence about the impact of ill mental health is now widely available, a comprehensive and inspiring Memorandum to deploy and change European mental health policies and practices is lacking. At present, the most promising and cost-effective way to maximise value of investments in mental health is implementing and using existing knowledge. Therefore, GGZ Nederland took the lead to promote and improve mental health in Europe by building an alliance of relevant stakeholders, including healthcare professionals, employers and social protection organisations. Objectives Such a European Implementation Program for Mental Health for the working population and the alliance that will rally for it, aim to: Ensure the right on access to mental health care across the European Union. Prevent, detect and treat psychosocial problems in an early stage, e.g. through regular screening of schoolchildren and employees on mental resilience and mental health problems. Scale up successful innovative approaches to mental health promotion at work, labour market retention and (re-)integration of people with mental health problems to competitive work. 13

14 Structure and consolidate information and evidence about the societal and economic benefits of mental health care in the health technology assessment of mental healthcare interventions. Broaden the scope of research and EU policy actions to the mental health of our (future) working population. E.g. in the upcoming EU strategy for Occupational Safety and Health Build cross-sectorial alliances in the spirit of Health in All Policies to develop and to implement effective long-term mental health strategies and to influence socio-economic determinants to keep people living in European communities healthy, active and productive as long as possible. Working document The context, rationale and set-up of the alliance and the European Implementation Program for Mental Health for the working population have been described in a working document. This document has been used in consultations of potential partners and has been elaborated and finetuned during meetings and partners Board consultations. Today, the document can be seen as the basis of the actions. It is still under construction. The partners have chosen for this form as to include as much as possible the interests of the partners and to keep the opportunity to attract more partners by taking on board related issues and interests. Since 2014, the working document has been endorsed by most of the alliance s partners. The Alliance The Alliance of the European Implementation Program for Mental Health for the working population is an informal co-operation of the partners on the specific issue of economic impacts of mental health and the role it plays to raise labour productivity as a possible exit strategy for the economic crisis. Its aim is to raise awareness for this issue and to stimulate policy development on an EU level in this domain. All partners recognize its importance and will try to incorporate it in their own, broader (public and mental) health policy agendas. Over the last year, the Alliance has attracted 5 partners. In meetings and discussions, the partners identified potential members of the informal alliance, which form the network of stakeholders. Partners Today, the Alliance is composed of 5 partners. These include: AEIP, the European Association of Paritarian Institutions, represents the social protection institutions jointly managed by social partners (paritarian institutions) and aims to promote the paritarian management of social protection at the EU-level. This has been its key objective since its founding in The values supported by AEIP are based on a balanced representation of employers and employees which is a typical feature of joint management (as well as social policy emanating from collective bargaining), solidarity and transparency; this approach is complementary to that applied within the social economy. Ultimately, its aim is the recognition of the status of the European Paritarian Institution of Social Protection as a structure enabling joint negotiations and joint management of cross-border collective agreements on pensions, health and providence schemes. Currently, AEIP is working on several political dossiers: Pensions; Health and Care; Other Social Benefits; Solidarity; Mobility; Social Dialogue; European Paritarian Institution. Working to inform the general public on the plight of people with mental health problems, Mental Health Europe also cooperates closely with the media, and is often featured in prominent media outlets in Brussels and beyond. 16 The alliance is an informal grouping and all partners agreed to actively participate in the group s meetings and in the development of the alliance and the European Implementation Program for Mental Health for the working population. It is expressively recognized that this initiative in no way binds the partners, nor that the partners will work exclusively on this specific issue. Network Apart from the partners, that take an active role in the alliance, there are many other stakeholders in the mental health and employment domain that have been contacted. These include Age Europe, ASPHER, DG Sanco, European Federation of Nurses, GAMIAN Europe, EUAPME (Small and Medium Enterprises), EuroHealthNet, European Health Forum Gastein, Workability Europe, Youth Forum, Meetings Over the last year, the alliance has met three times. On 25 September 2013, the Alliance met for the first time. Participants represented GGZ Nederland, EPHA and MHE and the main issue on the agenda was the agreement on the working document and the Alliance s statements. The meeting was meant to prepare the engagement of the partners and the agreement of the respective Boards of the partner organisations. The meeting was also an opportunity to learn from the partners and to discover their activities and interests. It was concluded that this set of partners was a good basis to continue the project and that a work program for the upcoming months was needed. On 17 December 2013, the Alliance met for a second time. In the meantime, AEIP had joined the Alliance and took actively part in this meeting. On the agenda was the involvement of the partners and the echoes of the respective Boards discussions and the partner-organisations endorsement. Most partners had positive feedback and only MHE had to make reservations towards the project. It was in this meeting that MHE informed about the critical situation of MHE and the fact that the Board due had not really discussed the document and its engagement in the Alliance. As regards any work program 2014, the participants were informed that MHE was not yet capable of engaging any further in this work. On 3 March 2014, a third meeting of the Alliance was organised and the main issue on the agenda was the launch and communication of the partnership and the European Implementation Program for Mental Health for the working population. With regards to the then upcoming European elections and the changes it could imply, it was agreed to postpone the launch and to find a suitable moment after summer break. Consultations In order to influence the European agenda and the European policies on mental health in general and mental well-being under the working population in particular, the partners of the alliance coordinate their inputs to consultations and policy proposals. Over the last year, the alliance has coordinated inputs on 2 public consultations. In their responses to both consultations, individual partners have taken on board the principles and issues of the alliance and stressed the need for more attention for and a stronger focus on mental health. The coordinated effort on the Consultation on a new European Strategy and Health at Work seemed to be successful as shown in the new Framework on Health and Safety at Work The consultation on Patient Safety led to the recognition that a mental health perspective was lacking. 16

17 May 2013, Consultation European Strategy on Safety and Health at Work The European Commission launched a public consultation to gather insights and contributions from the public to results of the evaluation of the European Strategy on Safety and Health at Work This should help identify current and future challenges in the occupational safety and health area, and identify solutions to address these challenges. Since 2002 the EU has fulfilled its political role in the area of Occupational Safety and Health by means of multi-annual Strategies formally endorsed by the Commission after consultations, and voluntarily implemented by Member States and stakeholders. Two strategies on health and safety at work have been adopted so far, the first in 2002, covering the period until 2006, the second in 2007 for the period until The Strategy set an ambitious goal for all Member States: to reduce accidents at work by 25 %. In addition, a number of specific objectives were identified, such as: improving and a better implementing the EU regulatory framework on occupational safety and health, adapting the framework to changes in the workplace, developing coherent national strategies adjusted to the specific context of each Member State and encouraging changes in behaviour and promotion of a preventive culture in all parts of society. In April 2011, the Commission published a mid-term review of the strategy. The final evaluation is now available online, based on a broad consultation process carried out among all the main interested parties in the area: occupational safety and health public authorities in the Member States, organisations representing employees and employers at EU and national levels, experts and other stakeholders in the field. June 2014, EU Strategic Framework on Health and Safety at Work (COM(2014)332) Based on the public consultation of 2013 to gather suggestions and contributions in the field of health and safety at work, the Commission launched the EU Strategic Framework on Health and Safety at Work In this, the Commission refers several times to mental health: identify and disseminate good practice on preventing mental health problems at work EU-OSHA. ESF funding will be available to support actions aiming inter alia to promote sustainable, quality employment and social inclusion through, in particular: o Prolonging healthier working lives through the development and implementation of measures to promote a healthy environment and mental well-being at work. This may be addressed through the investment priority on "active and healthy ageing". o Supporting the recruitment and return to work of people with a chronic or rare disease, disability or mental health disorder through integrated pathways combining various forms of employability measures such as individualised support, counselling, guidance, access to general and vocational education and training, as well as access to services, notably health and social services. Public policy in other areas can contribute to an improved working environment. Potential synergies with OSH policy need to be more actively explored. The key areas in this respect are as follows: o Education: Raising awareness of OSH starts at school. There have been recommendations to better reflect OSH issues in school curricula (especially in vocational training) as well as to better promote mental health and wellbeing. There have been successful pilot projects, but the results of these need to be better disseminated. Information and training for entrepreneurs need to continue; 17

18 o o o Research: OSH research priorities have been established, focusing on the impact of ageing, globalisation, new technologies and occupational, work-related diseases and disabilities. There is a need to better disseminate the results of this research and better reflect them in policy-making; Public health: Better coordination between policy-makers in this area is needed, in order to build on existing programmes and guidelines and create synergies. Cooperation with key stakeholders (end users, public authorities, industry) is needed through the Joint Action on mental health and well-being and within the European innovation partnership on active and healthy ageing. This will improve the conditions for uptake of innovation and investment in innovation; Changes in work organisation brought about by information technology developments, in particular those that allow for constant connectivity, open up enormous possibilities for flexible and interactive work processes. There is also increasing workforce diversity, as reflected in new atypical contractual arrangements and work patterns, and a higher job turnover associated with shorter job assignments, especially for younger workers. However, according to a recent Eurobarometer survey, workers consider stress to be one of the main occupational risks (53%), followed by ergonomic risks (repetitive movements or tiring or painful positions (28%)) and lifting carrying or moving loads on a daily basis (24%). Specific attention should be given to addressing the impact of changes in work organisation in terms of physical and mental health. In particular, women can face specific risks, such as musculoskeletal disorders or specific types of cancer, as a result of the nature of some jobs where they are over-represented. February 2014, Public consultation on Patient Safety and Quality of Care With this public consultation the European Commission sought an opinion of civil society on whether patient safety measures included in the Recommendation 2009 are implemented and contribute to improving patient safety in the EU; which areas of patient safety are not covered by the Recommendation and should be; what should be done at EU level on patient safety beyond the Recommendation; whether quality of healthcare should be given more importance in the future EU activities. Presentations The initiative for a European Implementation Program for Mental Health for the working population and the alliance have attracted lots of attention from interested parties.
